甲状腺转录因子1、细胞角蛋白7及抑癌基因p53在非小细胞肺癌中的表达及与临床病理的关系

摘要

目的：探讨甲状腺转录因子1（TTF-1）、细胞角蛋白7（CK7）以及抑癌基因 p53三项指标在非小细胞肺癌（ NSCLC ）中的表达及其与临床病理的关系。方法选取2011年2月至2012年12月恩施自治州中心医院肿瘤切除术后的NSCLC 患者44例病变组织作为观察组，另选35例肺良性病变组织作为对照组。通过免疫组织化学链霉亲和素生物素过氧化物酶复合物法检测 TTF-1、CK7及p53在NSCLC及肺良性组织中的表达水平，探讨 NSCLC中该3项指标与临床病理特征的相关性。结果44例NSCLC患者中的TTF-1、CK7、p53在阳性表达率分别为45.5％（20／44）、45.5％（20／44）、52.3％（23／44），明显高于对照组的2.9％（1／35）、5.7％（2／35）、2.9％（1／35）（P＜0.01）。男性患者TTF-1、CK7阳性率显著低于女性（ P＜0.05）；无吸烟史患者TTF-1、CK7阳性率显著高于有吸烟史患者（P＜0.05）；高分化患者TTF-1、CK7、p53阳性率显著低于中低分化患者（P＜0.05）。 N0患者TTF-1、CK7、p53阳性率显著低于 N1＋N2患者（P＜0.05）。生存时间≤3年患者 TTF-1、CK7、p53阳性率显著高于生存时间＞3年患者（P＜0.05）。结论 TTF-1、CK7以及p53在 NSCLC 组织中呈异常表达，与肿瘤的发生、发展相关。%Objective To explore the expression of thyroid transcription factor 1(TTF-1),cytokeratin 7 (CK7) and p53 in non-small cell lung cancer(NSCLC)and its relationship with the clinical pathology. Methods Lesion tissue of 44 patients with NSCLC lesion after tumor resection were selected in Enshi Autonomous Prefecture Central Hospital from Feb.2011 to Dec.2012 as observation group,and lesion tissue of 35 patients with benign pulmonary disease were included as control group .Immunohistochemistry streptavi-din-biotin peroxidase complex technology was used for detection of related protein TTF-1,CK7 and p53.The differences of related indicators between NSCLC and benign pulmonary disease were analyzed .The correlation between the expression of TTF-1,CK7 and p53 with the clinical pathological features in NSCLC were investi-gated.Results The positive expression rate of TTF-1,CK7 and p53 of NSCLC patients were 45.5%(20/44),45.5%(20/44),52.3%(23/44),respectively,which were significantly higher than those in the con-trol group[ 2.9%(1/35),5.7%(2/35),2.9%(1/35)](P <0.01).TTF-1,CK7 of male patients were significantly lower than the female(P<0.05);TTF-1,CK7 positive rate of the patients with no smoking his-tory were significantly higher than the patients with smoking history(P<0.05); the TTF-1,CK7,p53 posi-tive rate of the patients with high differentiation were significantly lower than the patients with low differentia-tion(P<0.05).The TTF-1,CK7,p53 positive rate of N0 patients was significantly lower than the N1 +N2 patients (P <0.05).TTF-1,CK7,p53 positive rate of patients with survival≤3 years were significantly higher than patients with survival >3 years patients (P <0.05).Conclusion TTF-1,CK7 and p53 are abnormally expressed in NSCLC tissues,which is related to the development and progression of tumor .
