反复着床失败的病因及治疗进展

摘要

辅助生殖技术的最终目的是不孕患者获得妊娠,而胚胎着床是成功妊娠的前提.胚胎着床是一个复杂的过程,要求囊胚与子宫内膜同步发育且功能协调,是母胎免疫耐受顺利建立的结果.反复着床失败(RIF)是辅助生殖技术的一大难题,根据胚胎着床机制将RIF的病因可分为三大类,即母体因素、胚胎因素及母胎界面免疫因素.因此,RIF的临床治疗方法也是从这三方面入手,其病因及治疗进展一直是生殖领域研究的热门课题,尤其是不明原因的RIF和免疫治疗.%The ultimate goal of assisted reproductive technology is for infertile patients to get pregnant,and embryo implantation is the prerequisite for successful pregnancy.Successful embryo implantation is the result of successful establishment of the maternal fetal immune tolerance,and needs blastocyst and endometrium synchronous development and coordinated function.Recurrent implantation failure (RIF) is a major problem for clinical workers in assisted reproductive technology.According to the embryo implantation mechanism,the causes of RIF can be divided into maternal factors,fetal factors and maternal fetal interface immune factors.Therefore,the clinical therapies of RIF are also from these three aspects,and the cause of repeated implantation failure and its treatment progress has been a hot topic in the field of reproductive research,especially unexplainable RIF and immunotherapy.
