网织红细胞参数用于鉴别诊断地中海贫血与缺铁性贫血的价值

摘要

目的：探讨网织红细胞（reticulocyte，Ret）参数用于鉴别诊断地中海贫血与缺铁性贫血（iron deficiency anemia，IDA）的价值。方法：研究病例来自2013年1月至2014年12月期间佛山市第二人民医院门诊初诊患者，其中，地中海贫血患者96例，IDA 患者65例，对照组为参加本院健康体检各项检查指标均正常者，共50例。采用Sysmex-XE5000全自动血细胞分析仪检测研究对象网织红细胞百分率（RET％）、网织红细胞绝对值（RET＃）、未成熟网织红细胞比率（IRF）、低荧光强度网织红细胞百分率（LFR％）、中荧光强度网织红细胞百分率（MFR％）、高荧光强度网织红细胞百分率（HFR％）和网织红细胞血红蛋白含量（Ret-He）七项指标。结果：地贫组和IDA组的RET%、RET #、IRF、MFR%、Ret-He有显著性差异（P<0.05）。地贫组和IDA组的LFR%、HFR %无显著差异（P>0.05）。地贫组和IDA组RET%、IRF、MFR%、Ret-He、LFR%、HFR %以及地贫组RET #与对照组比较有显著性差异（P<0.05），IDA组RET #与对照组比较无显著差异（P>0.05）。结论：网织红细胞相关参数中RET %、RET #、IRF、MFR %、Ret-He可用于鉴别诊断地中海贫血与IDA。%Objective:To explore the value of the reticulocyte(Ret) parameters in distinguishing between thalassemia anemia and iron-deficiency anemia(IDA).Methods:The subjects were selected from January,2013 to November,2014 in the second people,s hospital of Foshan, who were at their first visit to hospital. Among them there were 96 patients with thalassemia anemia, 65 patients with IDA, and 50 healthy controls. We detected the percentage of reticulocytes (RET%),absolute reticulocyte(RET#),immature reticulocyte fraction (IRF), low fluorescence intensity reticulocyte(LFR%),the fluorescence intensity reticulocyte(MFR%),high fluorescence intensity reticulocyte(HFR) and reticulocyte hemoglobin content(Ret-He) by Sysmex-XE5000 automated hematology analyzer.Results:There was significant difference between thalassemia anemia and IDA in RET%、RET #、IRF、MFR% and Ret-He (P<0.05),while there was no significant difference in LFR% and HFR %(P>0.05). Compared with health controls, thalassemia anemia and IDA have significant difference in RET%、IRF、MFR%、Ret-He、LFR%、HFR %. There was significant difference between thalassemia anemia and IDA in RET #, but no significant difference between IDA and health controls.Conclusions: RET %、RET #、IRF、MFR % and Ret-He can be used in differential diagnosis between thalassemia anemia and IDA.
