实时三维超声心动图在围术期结构性心脏病中的应用价值

摘要

Objective To evaluate the clinical value of real-time 3-dimensional echocardiography (RT-3DE) in diagnosing the perioperative period of structural heart disease. Methods 32 patients with structural heart disease were examined by real-time three-dimensional transthoracic echocardiography (RT-3DE TTE) and real-time three-dimensional transesophageal echocardiography (RT-3DE TEE). The features of lesions were revealed. The correlation of the measured value of intraoperative and ultrasound was evaluated. Results Although both RT-3DE TTE and RT-3DE TEE can construct the image of the pathological anatomy of structural heart disease, RT-3DE TTE prone to artifacts. The maximum defects area and the values of RT-3DE TEE imaging technique were more closer to the actual values. Conclusion The accuracy of RT-3DE TEE is higher than RT-3DE TTE in the evaluation of the structural heart disease during the perioperative period and it provide more reliable information to the treatment of surgical and medical intervention.%目的 评价实时三维超声(RT-3DE)成像在围术期结构性心脏病中的临床应用价值.方法 收集32例围术期结构性心脏病患者,应用RT-3DE经胸超声(TTE)和RT-3DE经食管超声(TEE)成像构建出病变的解剖特征,评价超声测值与手术测值的相关性.结果 RT-3DE TTE和RT-3DE TEE成像均能构建出结构性心脏病的病理解剖特征,但是RT-3DE TTE成像易出现伪像.在测量缺损最大值方面,RT-3DE TEE成像更接近术中测值.结论 在评价围术期结构性心脏病时,RT-3DE TEE的准确率高于RT-3DE TTE,为心脏外科手术和内科介入治疗提供的信息更为可靠.
