高场强MRI与常规超声诊断子宫肌瘤的对照研究

         

摘要

目的 探讨高场强MRI诊断子宫肌瘤的影像表现及与常规超声诊断价值的比较.方法 选取54例疑似子宫肌瘤患者,观察子宫肌瘤的高场强MRI表现,以术后病理结果为金标准,比较高场强MRI与常规超声的诊断差异.结果 手术及病理确诊子宫肌瘤49例,4例子宫腺肌症和1例子宫内膜息肉.高场强MRI显示患者子宫有不同程度增大和宫腔变形,肌瘤在T1WI上主要呈等信号或低信号,T2WI上呈低或不均匀信号病灶,边缘清晰,增强可见病灶信号强化或不强化;高场强MRI诊断准确度、灵敏度、特异度分别为98.15%、100.00%、80.00%;常规超声分别为87.04%、89.80%、60.00%,高场强MRI诊断准确度、灵敏度均高于常规超声(P<0.05);高场强MRI相关合并症检出率(93.33%)高于常规超声(60.00%)(P<0.05).结论 高场强MRI能较好显示子宫肌瘤的大小、形态变化,且诊断效果优于常规超声,可为临床诊断提供重要的影像支持.%Objective To explore the imaging findings of high-field intensity MRI in diagnosing hysteromyoma and compare its diagnostic value with that of conventional ultrasound.Methods A total of 54 patients with suspected hysteromyoma were selected to observe the high-field intensity MRI manifestations of hysteromyoma,and the pathological results were taken as the gold standard to compare the difference between high-field intensity MRI and conventional ultrasound.Results According to the operation and pathological results,hysteromyoma was confirmed in 49 cases,and adenomyosis in four cases and endometrial polyps in one case were confirmed.The high-field intensity showed that the uteruses had augmentation and uterine cavity deformation of different degrees,and myomas mainly showed equal or low signals for the focuses on T1WI while showing low or uneven signals on T2WI;the edges were clear,and the enhanced image showed enhancement or non-enhancement of the signals;the diagnostic accuracy,sensitivity,and specificity of the high-field intensity MRI were 98.15%,100.00%,and 80.00% respectively,while those conventional ultrasound were 87.04%,89.80% and 60.00%,respectively.The accuracy and sensitivity of high-field intensity MRI were higher than those of conventional ultrasound (P < 0.05);the ratio of associated complication detected by high-field intensity MRI was(93.33%),and this was higher than that of conventional ultrasound (60.00%)(P < 0.05).Conclusion High-field intensity MRI can show better the size and morphological changes of hysteromyoma,and the diagnostic effect is better than that of conventional ultrasound,which can provide important imaging support for clinical diagnosis.
