ICU病房应用无创呼吸机治疗心力衰竭合并呼吸衰竭的效果观察

摘要

Objective:To study the ICU application of noninvasive ventilator treatment of heart failure in clinical effect of combination of respiratory failure. Method:Patients with heart failure from 2010 to 2012 in our hospital ICU treatment combination of respiratory failure were selected as the research object,the observation group was carried out the routine drug treatment and 37 patients with noninvasive positive pressure ventilation therapy, the control group was carried out the routine drug treatment and 41 patients with low flow continuous oxygen therapy. 2 weeks after treatment,the clinical effect of the two groups was compared. Result:Within 2 weeks after treatmen,the comparison of the mortality rate and intubation rate showed that the observation group’s mortality,endotracheal intubation rate were far lower than those of the control group’s and observation group’s,and comparative advantage was obvious. Other performance before and after treatment,such as breathing,heart rate and pH,PaO2,PaCO2 blood gas results comparison showed that the observation group improved significantly better than control group. Conclusion:ICU application of noninvasive ventilator treatment of heart failure in the clinical effect of combination of respiratory failure,can effectively reduce the mortality,the rate of endotracheal intubation,and in other comparisons of heart failure and respiratory failure,it also has obvious advantages.%　　目的：探讨ICU病房应用无创呼吸机治疗心力衰竭合并呼吸衰竭的临床效果。方法：选取2010-2012年于本院ICU病房治疗的心力衰竭合并呼吸衰竭患者为研究对象，其中观察组患者37例实施常规药物治疗加无创正压通气治疗，对照组患者41例实施常规药物治疗加低流量持续吸氧治疗。比较两组患者治疗后2周时间内的临床效果。结果：治疗后2周时间内观察组患者死亡率、气管插管率远低于对照组，比较差异有统计学意义。其他治疗前后的表现如呼吸、心率以及pH、PaO2、PaCO2等血气结果比较中，观察组患者的改善情况要明显优于对照组。结论：ICU病房应用无创呼吸机治疗心力衰竭合并呼吸衰竭的临床效果显著，可以有效地减少死亡率、气管插管率，在心衰和呼吸衰竭的其他比较中，同样优势明显。
