无痛肠镜检查中使用酮咯酸氨丁三醇的效果评价

Objective:To evaluate the anesthetia effect and safety using ketorolac tromethamine combined with propofol intravenous anesthesia in painless colonoscopy examination.Method:120 patients which voluntary accept the painless colonoscopy examination were randomly divided into two groups.The cases in group A were undergoing propofol intravenous anesthesia only,while the cases in group B were intravenous dripped ketorolac tromethamine 30mg first then injected the propofol 5 minutes later.Memorize the changes of MAP,HR,RR and SpO2 in different time points.Two groups of patients with the dosage of propofol,the operation time,the physical activities in operation,the awake time,the pain score were compared.Result:All patients received successful operation. Also the patients’s MAP and HR decreased significantly during the operation in both group A and B,but there were no significant difference between two groups(P>0.05). The dosage of propofol,the pain score,the anesthetia effect of the patients’s in groups B were better than those in groups A(P<0.05).There were no obvious differences between two groups about the operation times and the awake time(P>0.05).Conclusion:The anesthetia effect were inhanced while use ketorolac tromethamine combined with propofol intravenous anesthesia in painless colonoscopy examination,this method did not increase the risk of anesthesia,the patients were satisfied with less abdominal pains,it was a safe and effective anesthesia method.%　　目的：评价酮咯酸氨丁三醇联合丙泊酚静脉麻醉用于无痛肠镜检查的麻醉效果及安全性。方法：选择120例自愿实施无痛肠镜检查患者，随机分为两组，A组仅使用丙泊酚，B组先静脉滴注酮咯酸氨丁三醇30 mg，5 min后静脉推注丙泊酚。记录两组患者在不同时间点的平均动脉压（MAP）、心率（HR）、呼吸（RR）及血氧饱和度（SpO2）数值，比较两组患者的丙泊酚用量、手术时间、术中肢动、清醒时间、疼痛评分。结果：所有患者均顺利完成手术，两组术中的MAP、HR均较术前显著降低（P<0.05），但两组间比较差异无统计学意义（P>0.05）。B组的丙泊酚用量、疼痛评分、麻醉效果显著优于A组（P<0.05）。两组手术时间、清醒时间比较差异无统计学意义（P>0.05）。结论：在无痛肠镜检查中联合应用酮咯酸氨丁三醇-丙泊酚能增强麻醉效果，不增加麻醉风险，术后腹痛减少，患者满意，是一种安全、有效的麻醉方法，值得在临床上推广应用。
