刮宫术联合宫腔放置水囊压迫法在子宫瘢痕妊娠处理中的应用价值

摘要

目的：探讨刮宫术联合宫腔放置水囊压迫法在子宫瘢痕妊娠处理中的应用价值。方法：选取2008年10月-2013年2月本院确诊治疗的子宫瘢痕妊娠患者86例，依据随机分配原则分为水囊组和对照组，两组患者均给予常规刮宫术、抗感染等综合治疗，刮宫术前，对照组患者给予甲氨蝶呤（30 mg/次）肌肉注射治疗，在此基础上，水囊组患者同时给予宫腔放置水囊压迫法治疗，统计分析所有患者治疗后刮宫术术中、术后出血量、血β-人绒毛膜促性腺激素（β-hCG）恢复正常、住院时间和术后不良反应发生情况和满意度。结果：水囊组患者术中、术后出血量、血β-hCG恢复正常和住院时间明显低于对照组，差异有统计学意义（P<0.05）；水囊组患者术后总不良反应发生率明显低于对照组，差异有统计学意义（P<0.05）；水囊组患者治疗后总满意度明显高于对照组，差异有统计学意义（P<0.05）。结论：刮宫术联合宫腔放置水囊压迫法可有效降低子宫瘢痕妊娠患者出血量，有助于加快患者的恢复，利于降低治疗后不良反应的发生和提高患者满意度，值得临床进一步推广。%Objective:To discuss the application value of compression caused by intrauterine placed water sac with curettage surgery method in the processing of uterine scar pregnancy.Method:86 patients with cesarean scar pregnancy were selected from October 2008 to February 2013 in our hospitals,according to the random distribution, all patients were divided into water capsule group and the control group,two groups of patients were given conventional curettage surgery,resistance to infection,such as comprehensive treatment,and curettage before operation,the control group patients given methotrexate(30 mg/day)intramuscular injection treatment,on this basis,the water sac patients caused by intrauterine placed water sac compression method was given at the same time,the statistical analysis of all of the patients after treatment and curettage surgery intraoperative and postoperative blood loss and blood beta hCG returned to normal,length of hospital stay and postoperative adverse reaction happening and satisfaction.Result:Intraoperative and postoperative blood loss and blood beta hCG back to normal and hospitalization time of water capsule group were significantly lower than those of the control group,the difference was statistically significant(P<0.05).Postoperative total incidence of adverse reactions of water capsule group was obviously lower than that of the control group(P<0.05).Total satisfaction of water capsule group was obviously higher than that of control group(P<0.05).Conclusion:Compression caused by intrauterine placed water sac with curettage surgery method can effectively reduce the bleeding,uterine scar pregnancy patients can help speed up the recovery of patients,to reduce the occurrence of adverse reactions after treatment and improve patient satisfaction,it’s worth for further clinical promotion.
