探讨手术患者凝血功能指标监测变化的临床意义

摘要

目的：对手术患者凝血功能指标监测变化的临床意义进行分析探讨。方法：选取2013年7-10月在本院实施大手术的50例患者以及50例正常体检者作为研究对象，分别设为观察组和对照组，观察组在手术前以及术后第1、3、5天实施采血，并对凝血酶原时间、凝血活酶时间以及纤维蛋白原等指标进行监测，同时对两组患者的监测结果进行对比分析。结果：观察组在术后第1天纤维蛋白原及D-二聚体等指标均明显高于对照组，差异均有统计学意义（P<0.01）；当术后第3天进行监测时，以上观察指标虽然有所下降，但依然明显高于对照组，差异均有统计学意义（P<0.01）；术后第5天对上述指标进行监测时发现，观察组已恢复至正常水平。而在凝血酶原时间和活化部分凝血活酶时间上，两组在不同时间段上比较均无明显差异（P>0.05）。结论：实施手术治疗的患者，其血液处于高凝状态，因此，对其进行凝血功能监测以便及时给予抗凝等防治，能够促进患者恢复。%Objective:To investigate and analyze the clinical significance of blood coagulation function in patients with operation monitoring changes.Method:50 large operation patients and 50 normal persons were selected in our hospital from July 2013 to October 2013 as research objects,which were regarded as the observation group and the control group,the observation group was carried out blood sampling before the operation and 1,3,5 days after surgery,and the prothrombin time,clotting enzyme and fibrinogen and other indicators were monitored,and at the same time,the monitoring results of two groups of patients were compared and analyzed.Result:The fibrinogen and D-dimers and other indicators postoperative first day in observation group were significantly higher than those in control group,the differences were statistically significant(P<0.01).When the third day after operation were monitored,the above observation indexes were declined,but still significantly higher than those in control group,the differences were statistically significant (P<0.01).It found that the above indexes after fifth days were monitored,the patients in observation group had recovered to normal level.While in the prothrombin time and activated partial thromboplastin time,two groups of patients in different time comparison showed no significant difference(P>0.05).Conclusion:Implement the operation treated patients, the blood is in hypercoagulable state,therefore,the monitoring of coagulation function in order to provide timely anticoagulation control,promote the patient recover.
