后程加速超分割三维适形放疗联合卡培他滨对中晚期食管癌的疗效研究

摘要

To investigate the effect analysis of late course accelerated hyperfractionated three-dimensional conformal radiotherapy combined with capecitabine in treatment of patients with local advanced esophageal cancer.Method:90 patients with advanced esophageal carcinoma were treated in our hospital were collected,they were randomly divided into the combined treatment group(n=45)and the radiotherapy group(n=45). The radiotherapy group was treated with late course accelerated hyperfractionated three-dimensional conformal radiotherapy for treatment,and the combined treatment group was combined with capecitabine in basic of the radiation group. The clinical effect of the two groups was observed. Result:The two groups all achieved the ideal effect,no obvious difference. The radiotherapy group total effective rate was 88.89%,the combined treatment group was 91.11%(P>0.05). The combined treatment group’side effects attack rate was 31.11%,and was lower than the radiotherapy group(P<0.05). At the same time,all the patients were followed up for three years,the recurrence rate and mortality rates in the combined treatment group were lower than the radiotherapy group(P<0.05).Conclusion:Late course accelerated hyperfractionated three-dimensional conformal radiotherapy combined with capecitabine in the treatment of late the esophagus and achieved good effect,can effectively improve the clinical symptoms,prolong the survival time,improve the quality of life of patients,is worthy to be popularized clinically.%目的：探讨对局部中晚期食管癌患者采用后程加速超分割三维适形放疗联合卡培他滨的治疗效果分析。方法：采集本院收治的中晚期食管癌患者90例，按随机数字表法分为放疗组和联合治疗组各45例。放疗组采用后程加速超分割三维适形放疗进行治疗，联合治疗组在放疗组的治疗基础上联合给予卡培他滨。观察两组的临床效果。结果：治疗后，两组均取得较为理想的效果，无明显差异；放疗组的总有效率为88.89%，联合治疗组为91.11%，差异无统计学意义（P>0.05）。联合治疗组副反应发作率为31.11%，低于放疗组（P<0.05）。随访3年，联合治疗组的复发率及死亡率均低于放疗组（P<0.05）。结论：后程加速超分割三维适形放疗联合卡培他滨治疗中晚期食道癌取得了良好疗效，有效地改善了患者临床症状，延长了生存期，提高了患者生存质量，值得临床上广泛的推广。
