经脐单孔与三孔法腹腔镜胆囊切除术的临床疗效对比研究

摘要

Objective To compare the advantages and disadvantages of transumbilical single port and the three port laparoscopic cportcystectomy.Methods A retrospective analysis of 98 cases of laparoscopic cportcystectomy in patients with clinical data,including the use of transumbilical single port method in 48 cases,50 cases of three port method,comparative analysis,operation time of two groups of bleeding,postoperative pain,complications,operation time,hospital food time,the cost of treatment, cosmetic satisfaction scores of abdominal wall.Results The operation time was longer than three single umbilical port group ( <0.05),the degree of pain of single port method in patients with abdominal wall cosmetic satisfaction scores were better than the three port method ( <0.05),the remaining patients between two groups are almost the same,no significant dif erence ( >0.05).Conclusion Compared with three ports LC,although the operation of single port method because of the dif iculty increases with longer operation time.It has the advantages of bet er cosmetic results of the pain and postoperative.%目的：对比分析经脐单孔与三孔法腹腔镜胆囊切除术的优缺点。方法回顾性分析98例行腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)患者的临床资料，其中采用经脐单孔法48例，三孔法50例，对比分析两组的手术时间、术中出血量、术后疼痛、手术并发症、进食时间、住院时间、治疗费用、腹壁美容满意评分等。结果经脐单孔法手术时间长于三孔法组(<0.05)，单孔法患者疼痛程度与腹壁美容满意评分均优于三孔法患者(<0.05)，其余方面两组比较几乎相同，无显著统计学差异(>0.05)。结论与三孔法LC相比，虽然单孔法手术因难度增加而手术时间更长。但其具有减轻疼痛程度及术后美容效果更好的优点。
