炔雌醇环丙孕酮联合二甲双胍对多囊卵巢综合征患者内分泌及糖脂代谢的影响

     

摘要

目的:分析炔雌醇环丙孕酮联合二甲双胍对多囊卵巢综合征内分泌及糖脂代谢的影响。方法选择在本院接受治疗的多囊卵巢综合征患者为研究对象,采用随机数字表法分为观察组和对照组,观察组接受炔雌醇环丙孕酮联合二甲双胍治疗,对照组接受炔雌醇环丙孕酮治疗。比较2组患者治疗后内分泌激素水平、血糖血脂水平及身体一般指标差异。结果观察组接受炔雌醇环丙孕酮联合二甲双胍治疗后,其月经恢复率、血清促卵泡激素( FSH )、黄体生成素( LH)、睾酮( T)、胰岛素( INS)、空腹血糖( FPG)、胰岛素抵抗( HOMA-IR)等内分泌水平,总胆固醇( TC)、甘油三酯( TG)、低密度脂蛋白胆固醇( LDL-C)、高密度脂蛋白胆固醇( HDL-C)、载脂蛋白A( ApoA)、载脂蛋白B ( ApoB)等血脂水平,体质指数( BMI)、腰臀比( WHR)值、多毛评分及痤疮评分值等身体状况的改善均优于对照组患者(P<0.05)。结论炔雌醇环丙孕酮联合二甲双胍用于多囊卵巢综合征患者的治疗,可以有效优化患者的激素水平,降低高脂血症,对疾病康复具有积极意义。%Objective To evaluate the effects of cyproterone acetate and ethinylestradiol combined metformin on the endocrinological function and glucose and lipid metabolism in polycystic ovary syndrome ( PCOS) patients.Methods P-COS patients who were admitted into our hospital were included in the current study and randomly divided into a control group and an observation group.The control group was treated with cyproterone acetate and ethinylestradiola alone, while the observation group was administrated with a combination of cyproterone acetate and ethinylestradiola and metformin. Both groups were compared for the levels of hormone, blood glucose and lipid and physical condition after treatment.Re-sults After the combined therapy, the observation group showed improved changes than the control in menstruation re-covery rate, follicle-stimulating hormone ( FSH) , luteotropic hormone ( LH) , testosterone ( T) , insulin ( INS) , fasting plasma glucose (FPG), homeostasis model assessment-insulin resistance ( HOMA-IR) as well as total cholesterol (TC), triglyceride (TG), low-density lipoprotein -cholesterol (LDL-c), high-density lipoprotein-cholesterol ( HDL-c) , lipoprotein A ( ApoA) , lipoprotein B ( ApoB) and body mass index ( BMI) , waist-to-hip ( WHR) rati-o, hirsutism score, and acne score (P<0.05).Conclusions Treatment of PCOS patients with cyproterone acetate and ethinylestradiol combined metformin can effectively improve endocrinological function and glucose and lipid metabolism, which is beneficial for recovery.
