Objective To compare the clinical efficacy of one-stage and two-stage skin implant with vacuum sealing drainage(VSD)technology in the treatment of lower leg and foot degloved skin injury.Methods From November 2010 to November 2012,29 cases of lower leg and foot degloved skin injury were treated with skin implant and VSD pressure drainage,15 patients in group A were treated with one-stage skin implant and VSD,14 cases in group B were treated with two-stage skin implant with VSD.Results The average operation time was(119.33±6.35)min in group A and(139.64±5.31)min in group B;the average healing time was(7.40±2.03)days in group A and(9.43±3.01)days in group B;the average postoperative skin survival rate was(88.47±7.63)％in group A,(78.07±17.44)％in group B,there was significant difference of survival rate between the two groups(P<0.05).The range of joint motion was(114.40±19.35)°in group A and(116.29±18.76)° in group B,no significant difference of joint motion was found between the two groups(P>0.05).Conclusion Compared with two-stage in situ skin implant,one-stage in situ skin implant with VSD technology could shorten the operation time and healing time,increasing the skin survival rate.%目的 比较1期原位皮肤回植联合负压封闭引流技术(VSD)与2期植皮治疗小腿及足部皮肤脱套伤的临床疗效.方法 选择2010年11月-2012年11月新疆医科大学第一附属医院收治的29例小腿及足部皮肤脱套伤患者,其中1期原位皮肤回植联合VSD技术15例(A组),2期植皮技术14例(B组),比较2组患者的手术时间、皮肤愈合时间、首次植皮存活率、关节活动度(膝及踝关节)以及皮肤愈合情况.结果 A组与B组的手术时间分别为(119.33±6.35)、(139.64±5.13)min,愈合时间分别为(7.40±2.03)、(9.43±3.01)d,首次植皮存活率分别为(88.47±7.63)、(78.07±17.44)％,差异均有统计学意义(P<0.05).A组和B组关节屈伸活动度分别为(114.40±19.35)°和(116.29±18.76)°,差异无统计学意义(P>0.05).末期随访皮肤愈合情况:2组植皮区域皮肤色泽红润,瘢痕较小,外观满意,皮肤浅感觉及弹性均良好.结论 1期原位皮肤回植联合VSD技术与2期植皮治疗小腿及足部皮肤脱套伤均能取得较好的临床疗效,但1期原位皮肤回植联合VSD技术在手术时间、皮肤愈合时间、首次植皮愈合率上更具有优势.
