乳腺X线钼靶表现与ER、PR和HER2表达的乳腺癌亚型的病理对照研究

     

摘要

目的 探讨数字化钼靶摄影乳腺癌的相关影像特征和乳腺癌生物学指标雌激素受体(ER)、孕激素受体(PR)、人类表皮生长因子(HER2)的相关性.方法 依据乳腺影像报告和数据系统(BI-RADS)X线部分评估乳腺浸润性导管癌的X线征象,通过免疫组化取得ER、PR和HER2表达结果,运用χ+2验或者Fisher精确概率法检验分析浸润性乳腺癌的生物学指标与乳腺X线相关指标的相关性.结果 单纯肿块多见于Luminal A型;单纯钙化多见于Luminal B型和HER2过表达型;局部致密伴钙化及结构扭曲伴钙化病变多见于HER2过表达型.HER2过表达型乳腺癌钙化多呈细小多型性,区域性分布,Luminal A型乳腺癌钙化以不定型和细小多型性多见.结论 乳腺癌的数字化X线表现可一定程度上预测其分子亚型,为临床治疗及判断其预后提供影像学方面的根据,具备潜在的临床推广应用价值.%Objective To investigate the correlation between mammographic features and biological markers of breast cancer,including estrogen receptor(ER),progesterone receptor(PR)and Human epidermal growth factor(HER2).Methods According to breast imaging report and data system(BI-RADS),the X-ray features of breast invasive ductal carcinoma was evaluated;The expressions of ER,PR and HER2 were obtained by immunohistochemistry;The correlation between the expression of biomarkers of invasive breast cancer and the mammographic features was analyzed by Chi-square or Fisher′s exact probability test.Results Breast lumps were more commonly present in Luminal A subtype,calcification was mostly present in Luminal B subtype and HER2 positive subtype;local compact with calcification and structural distortion with calcification was more common present in HER2 positive subtype.The calcification of breast cancer with HER2 positive type was small,multiple,regional distribution,whereas the calcification of Luminal A type was presence with amorphous and small.Conclusion The digitalized mammographic detection in a way could predict and diagnose the subtypes of breast cancer,providing the imaging evidence for clinical therapy and diagnosis,which was therefore of potential clinic application.
