小诺米星在新生儿体内的药物动力学 及其临床意义

     

摘要

目的 研究新生儿小诺米星药物动力学,并获药动力学参数。方法 对8例新生儿患者按4mg/(kg.d)静脉点滴小诺米星,用药后0.5、1、2、4、6、8、10h各静脉采血0.5~1mL,用荧光光度法测定血药浓度。用3P87程序计算药物动力学参数。结果 小诺米星在新生儿体内呈二室开放式模型。主要药动学参数:平均血药峰浓度(Cmax)为(3.86±1.2)mg/L,消除半衰期(T1/2β)(5.32±1.8)h,清除率(CL)为(0.08±0.03)mg/L,曲线下面积(AUC)为(29.7±10.47)mg/(L.h),10h血药浓度为(1.02±0.418)mg/L。结论 药动学参数与儿童、成人比较,有显著性差异,小诺米星在新生儿血液和组织中作用较持久,建议每日一次给药方案%Objective To study the pharmacokinetic of newborn,thepharmacokinetic parameters of MCR in newborn were obtained.Methods Eight newborns were infused with MCR (4mg/kg,once daily),after infused,serum concentration of MCR at 0.5、1、2、4、6、8 and 10h were determined by spectrofluorometry.The pharmacokinetic parameters were calculated by 3P87 computer program.Results MCR after iv gtt in newborns was fit in with two compartment model.The main pharma-cokinetic parameters were Cmax (3.86±1.2)mg/L,T1/2β(5.32±1.8)h,CL(0.08±0.03)mg/L,AUC(29.7±10.47)mg/(L.h).After infused for 10h,serum concentration of MCR was (1.02±0.418)mg/L.Conclusion There was markedly variant in newborn,children and adult.MCR has long role in blood and tissue of newborn,the regimen of qd was suggested.
