加压骨钢针治疗第一掌骨基底部骨折

摘要

目的：探讨加压骨钢针治疗第一掌骨基底部骨折的手术方法及临床疗效。方法对36例第一掌骨骨折患者行切开复位加压骨钢针内固定术，术后对第一腕掌关节功能、疼痛及拇指捏力、握力进行评估。结果34例获得随访，时间4～12个月；影像学显示均达到骨性愈合，愈合平均时间为7周。随访时患侧拇指握力、捏力、总主动活动度（TAM）和外展角较健侧差异均无统计学意义。结论采用加压骨钢针治疗第一掌骨基底部骨折，是一种简单有效的内固定手术方法。%Objective To explore the operation method and the clinical curative effect of the treatment of fracture of the base of the first metacarpal with pressurized bone screws.Methods Thirty-six patients with the fracture of base of the first metacarpal were treated by the fixation with pressurized bone screws.The function of the first metacarpal,pains,thumb pinch strength and grip strength were evaluated after operation.Results Thirty-four patients were followed up,with time ranged from 4 to 12 months.X ray examination indicated bone healing in all fractures.There was no statistical difference in the aspects of thumb pinch,grip strength,total active motion(TAM) and abduction angle.Conclusion Using pressurized bone screws is a simple and effective method for treating the fracture of the base of the first metacarpal.
