纽扣双钢板与锁骨钩钢板治疗急性TossyⅢ型肩锁关节脱位的临床对比

摘要

Objective To explore the advantages and disadvantages of the therapeutic effect of TossyⅢacromioclavicular joint dislocation with double-endobutton plate and clavicle hook plate.Method A retrospective analysis was performed on 37 patients with TossyⅢacromioclavicular joint dislocation in our hospital from January 2013 to June 2015[Age 22~67 years old, average (40.43±9.45) years old; male 21 cases, female 16 cases]. Seventeen cases of TossyⅢacromioclavicular joint dislocation were treated with double-endobutton plate(endobutton group), and other 20 cases were treated with clavicle hook plate (hook group). The operation time, incision size, the amount of bleeding during operation, and the UCLA function score of two groups of patients were observed and recorded.Result There is no significant difference of operation time between endobutton group and hook group, but the differences of the incision size, amount of bleeding and postoperative UCLA shoulder score, between the two groups are statistically significant.Conclusion It is worthy of being promoted as double-endobutton plate has a better effect than the clavicular hook plate in the treatment of TossyⅢ acromioclavicular joint dislocation.%目的：对比运用纽扣双钢板与锁骨钩钢板治疗TossyⅢ型肩锁关节脱位疗效，探讨两者的优劣。方法对本院2013年1月至2015年6月手术治疗TossyⅢ型肩锁关节脱位37例患者进行回顾性分析。患者年龄22~67岁，平均（40.43±9.45）岁；男性21例，女性16例，其中采用纽扣钢板治疗17例（纽扣组），锁骨钩钢板治疗20例（锁骨钩组）。观察并记录两组患者的手术时间、切口大小、术中出血量并统计两组患者的UCLA功能评分。结果纽扣组与锁骨钩组在手术时间上，两组差异无统计学意义；纽扣组与锁骨钩组在切口大小、术中出血量以及术后肩关节UCLA功能评分方面两组差异均有统计学意义。结论临床上采用纽扣钢板比锁骨钩钢板治疗TossyⅢ型肩锁关节脱位，具有更好的疗效，值得推广。
