消化道穿孔的多层螺旋CT诊断价值

摘要

目的：总结消化道穿孔的多层螺旋CT表现，分析其诊断价值。方法：选择30例急性胃肠道穿孔患者为研究对象，将其在手术前的多层螺旋CT检查资料和手术结果进行对照。结果：30例患者中胃小弯前壁穿孔8例，十二指肠球部穿孔6例，胃窦前壁穿孔6例，肠穿孔4例，幽门前壁穿孔4例，阑尾穿孔2例。观察结果显示胃肠道穿孔CT直接表现有胃肠道管壁形状不规则，管壁周围脂肪层模糊；间接表现有腹腔内有游离气体离散，还存在有一些其他的并发症。结论：对于胃肠道穿孔后临床症状比较轻，以及X射线透视膈下无游离气体的患者具有一定的诊断价值，并且比其他影像学检查的效果更好。%AIM:To conclude multislice CT manifestation of di-gestive tract perforation,and analyze its value.METHODS:Preoperative MSCT data of 30 cases the sites of gastrointestinal perforation were different,which comprised an anterior wall of gastric antrum (n=8),anterior wall of pylorus (n=6),anterior wall of gastric lesser curvature (n=6),duodenal bulb (n=4), bowel (n=4)and appendix (n=2).On MSCT images,the di-rect signs of acute gastrointestinal perforation were found to be ir-regular shape of the affected walls and hazy fatty layers in the pe-riphery of perforation,while indirect signs of that included intra-abdominal scattered and free gas and a series of the complications secondary to the perforations.CONCLUSION:MSCT is valua-ble,especially in the diagnosis of posterior gastric wall perforation and superior to other imaging model lities.
