目的 探讨体外受精-胚胎移植(IVF-ET)周期中,影响胚胎着床的原因,为单胚胎移植,减少多胎妊娠寻找理论依据. 方法 IVF-ET 1,193个周期,根据胚胎种植率分为三组:A组种植率0％；B组种植率50％；C组种植率100％.将三组的胚胎形态及临床各参数比较分析. 结果 1,193个周期鲜胚移植共984个周期,三组注射人绒毛膜促性腺激素(HCG)日血清黄体生成素(LH)、雌二醇(E2)水平、子宫内膜厚度有差异,但无统计学意义(P＞0.05)；三组HCG日血清孕酮(P)水平分别为(2.69±1.00) nmol/L、(2.64±0.97) nmol/L、(2.51±0.94) nmol/L,其中A与C组差异有统计学意义(P＜0.05).三组移植胚胎形态也存在明显的差异,D3的8A级胚胎的比率分别为31.53％、50.53％、60.20％.A组中,D3的6细胞和少于6细胞的移植胚胎率明显显著高于另外两组(P＜0.01). 结论 在注射HCG日血清P水平较低,胚胎8A的比率较高,年龄较轻的患者,其胚胎的着床率较高,是进行单胚胎移植的适宜人群.%Objective: To analyze the influence factors of embryo implantation in vitro fertilization-embryo transfer(IVF-ET).rnMethods: Nine hundred and eighty-four cycles of infertile patients who underwent IVF-ET in Zhuhai Maternal and Children's Hospital and Health Institute from January 2011 to December 2011 were analyzed retrospectively. The patients were divided into three groups: group A (implantation rate 0%,n = 344), group B(implantation rate 50% ,n = 285)and group C(implantation rate 100% ,n = 255). The difference of embryo morphology and clinical variables were compared and analyzed among the three groups.rnResults: No statistically significant difference was found in the levels of luteinizing hormone(LH)and estradiol(E2)in serum,and endometrial thickness on HCG day among three groups(P>0. 05). The levels of progesterone (P)on HCG day in group A,B,C were (2. 69 ± 1. 00) nmol/L, (2. 64 ± 0. 97) nmol/L, (2. 51 ± 0. 94)nmol/L in serum respectively. The difference between group A and group C was statistically significant(P<0. 05). The embryos rates of grade 8A in group A,B and C were 31. 53%, 50. 53% and 60. 20% respectively. The difference was statistically significant among the three groups(P<0. 01). The embryos rate of 6-cell and less than 6-cell in group A was significantly higher than the other two groups (P<0. 01).rnConclusions; The implantation rate is higher when the serum P value on HCG day is lower,the rate of embryos with grade 8A is higher,and the age is lower. Such kind patients are suitable for single embryorntransfer.
