目的 研究双歧杆菌三联活菌的综合疗法对肝硬化胃食管静脉曲张出血的防治作用.方法 临床收集120例肝硬化食管胃静脉曲张患者,按照随机数字方法分成2组,每组60例,其中对照组予以护肝、利尿、降低门脉高压、输注白蛋白和新鲜血浆等传统方法治疗;研究组则在上述治疗方法的基础上,同时给予双歧杆菌三联活菌和莫沙必利治疗.治疗后,比较分析两组患者的相关临床资料.结果 治疗后,研究组的TNF-α、NO及LPS水平均较对照组显著降低(P<0.05),同时研究组的CO、CI、PVF及EVD大小也均较对照组显著减小(P<0.05).经比较,对照组与研究组的总有效率分别48.33%和81.67%,二者具有显著性差异(P<0.05).结论 双歧杆菌三联活菌的综合疗法可以显著降低对门脉高压不利的相关细胞因子水平以及显著改善其血流动力学效应,对肝硬化食管胃静脉曲张出血具有很好的防治作用.%Objective To investigate the preventive and therapeutic effect of bifidobacterium trigeminy living bacterium therapy on cirrhosis complicated with gastroesophageal varices bleeding. Methods Two hundred and twenty patients of cirrhosis complicated with gastroesophageal varices bleeding were retrospectively reviewed and randomized equally into normal groups and study group. In control group, the patients were treated with traditional methods including diuresis, protection of liver,infusion of albumin and fresh plasma and decrease of portal hypertension. In study group, bifidobacterium trigeminy living bacterium and mosapride was administrated orally for 12 weeks based on the traditional methods mentioned above. After the treatment, the clinical data of the two groups were compared. Results After treatment, the levels of TNF-α, NO and LPS in study group were significantly lower than those in control group ( P ＜ 0.05 ) , meanwhile, the CO, CI, PVF and EVD size in study group were also decreased significantly than those in control group ( P ＜ 0.05 ). There were significant differences between the two groups in the total effective rate,48. 33％ vs. 81.67％ (P ＜0.05). Conclusion The combination therapy of bifidobacterium trigeminy living bacterium can significantly reduce the adverse cytokine levels related to portal hypertension and improve the hemodynamic effects;therefore it is conducive to the prevention of gastroesophageal varices bleeding in cirrhosis.
