湛江市391名RhD阴性无偿献血者的抗原表型与不规则抗体调查

摘要

目的 分析湛江地区Rh阴性献血者抗原表达和不规则抗体的抗体分布情况,为临床安全输血提供依据.方法 应用微量板盐水法筛选RhD阴性献血者,用间接抗人球蛋白试验(IAT)确认及鉴定C、c、E、e抗原表型.用筛选细胞对不规则抗体进行筛选,谱细胞进行鉴定.结果 199528名献血者初筛出391名RhD阴性(0.2%);其中ccee 181 名(46.3%)、Ccee 162 名(41.4%)、CCee 29 名(7.4%)、CcEe4名(1%)、ccEe9名(2.3%)、CCEe2名(0.5%)、ccEE2名(0.5%)和CCEE2例(0.5%);确认为弱D/部分D 4名.391份Rh阴性标本检出不规则抗体的标本有2例(0.5%),全为IgG类抗-D抗体,均有妊娠史.结论 湛江市Rh阴性无偿献血者血清学具有多种表型,以ccee表型为主;有少数人产生不规则抗体,大多与有妊娠史相关.%Objective To analyze Rh D negative phenotypes and irregular antibody distribution in blood donors in Zhanjiang area,to provide reference for safe transfusion.Methods RhD phenotype was initially screened by routine serological test.A1l of weak D or partial D phenotype was confirmed by indirect anti-globulin test (IAT).The C,c,E,e phenotypes and Irregular antibody were detected.Results 391 cases of RhD(-) were confirmed in 199528 donors(0.2%).The CcEe phenotypes of Del were 181 ccee (46.3%),162 Ccee (41.4%),29 CCee(7.4%),4 CcEe(1%),9 ccEe (2.3%),2 CCEe (0.5%),2 ccEE (0.5%) and 2 CCEE(0.5%).4 samples were weak D or partial D.2 cases were positive for irregular antibody in 391 RhD (-),all of them anti-D antibody of IgG type and they experienced pregnant.Conclusions The Rh D(-) phenotype of donors in Zhanjiang area was variety,primarily phenotype was ccee.A few donors had irregular antibodies which probably due to pregnancies.
