前路减压融合术治疗陈旧性胸腰椎骨折伴截瘫围手术期护理总结

摘要

目的：总结前路减压融合术治疗陈旧性胸腰椎骨折伴截瘫的围手术期护理方法。方法：32例行前路减压椎体间植骨融合内固定术，围手术期给予精心护理，早期功能锻炼，加强营养，严防并发症的发生。随访10～39个月，平均19.2个月。结果：神经功能恢复满意率83.3%，按Frankel分级有1～3级改善，平均改善1.8级。结论：对于脊髓前方有明显压迫物存在的陈旧性胸腰椎骨折伴截瘫患者，行前路减压融合术截瘫恢复率高，及时有效的护理是康复的关键。%Objective:To investigate the perioperative period nursing strategy of anterior decompression fusion therapy for old thoracolumbar fractures complicated with paraplegia.Methods:32 patients with old thoracolumbar fractures complicated with paraplegia were received anterior decompression together with internal fixation and bone graft fusion.In the perioperative period,all cases were given to intensive care,an active line of early functional exercise,better nutrition supply, and prevent complications.All of the 32 patients were followed up for a mean of 19.2 months (rang from 10 to 39 months) postoperative.Results:All cases acquired a follow-up 6~38months.According to Frankel classification,all patients improved from 1 to 3 grades (a mean of 1.8 grades),total effective rate being 83.3%.Conclusions:Anterior decompression fusion for old thoracolumbar fractures complicated with paraplegia patients with obvious compressive object in front of the spinal cord leads to high-rate recovery for patients with paraplegia.Timely and effective nursing measure is the key to this kind of patient rehabilitation.
