Objective To study the accuracy and advantage of color Doppler ultrasound in the diagnosis of common bile duct stones. Methods Three hundred and eighty cases of patients with bile duct lesions from April 2010 to April 2015 in our hospital were selected, 280 cases with biliary calculi were confirmed by surgery and pathology, who underwent Doppler color ultrasound examination before surgery, the results were compared with the re-sults of surgery and pathology, by comparison with diagnosis of other diseases in the lower segment of the bile duct by Doppler color ultrasound, the advantages and the accuracy of ultrasound in the diagnosis of bile duct stones were dis-cussed. Results In contrast to the diagnosis of gallstones, gallstone diagnosis rate for calculus of intrahepatic duct and common bile duct stones were higher(P<0.05); While in contrast to other common bile duct disease , diagnosis rate of cocommon bile duct cancer, ampullary carcinoma, pancreatic head carcinoma diagnosed rate were significantly lower than those of common bile duct calculi diagnosis rate (P<0.05). Conclusion Color Doppler ultrasound for the diagnosis of biliary calculi, especially for the diagnosis of bile duct stones, has great advantages.%目的：研究多普勒彩色超声对诊断胆总管下段结石的准确性及优势。方法选取380例我院2010年4月至2015年4月收治的胆管病变患者，经手术病理确诊，其中胆系结石280例，对患者术前进行多普勒彩色超声检查，就所得结果与手术病理结果作对比，对照多普勒彩色超声诊断胆总管下段其他疾病的诊断，得出超声在诊断胆总管下段结石的优势及准确性。结果在对比胆结石中的诊断率发现，胆囊结石的确诊率对于肝内胆管结石与胆总管下段结石来说均较高（P＜0．05）；而在于其他胆总管下段疾病的对比发现，胆总管下段癌、壶腹癌、胰头癌的确诊率均明显小于胆总管下段结石确诊率（P＜0．05）。结论彩色多普勒超声对于胆系结石的诊断，特别是胆总管下段结石的诊断极具优势。
