CT与MRI在早期股骨头坏死诊断中的临床应用价值

         

摘要

目的 探讨CT,MRI影像学检查在早期股骨头坏死诊断中的应用价值.方法 选取2016年1月-2017年5月收治的早期股骨头坏死56例(共98髋),均进行多层螺旋CT及MRI检查,比较两种方法对各临床分期股骨头坏死的检出率及早期征象的检出率.结果 MRI对股骨头坏死总检出率高于CT,且MRI对I~II期股骨头坏死检出率高于CT,存在统计学差异(P<0.05);在I~II期股骨头坏死患者中,MRI对线样征、骨髓水肿、骨小梁模糊的检出率高于CT,存在统计学差异(P<0.05);两种检查方法对局部囊变检出率比较无统计学意义(P>0.05).结论 MRI在早期股骨头坏死诊断中要优于CT,尤其是骨髓水肿、线样征、骨小梁模糊等典型征象,具有良好的临床诊断价值.%Objective To explore the clinical value of CT and MRI imaging in the diagnosis of early femoral head necrosis. Methods Selected 56 cases of early femoral head necrosis (98 hips) in our hospital, from January 2016 to May2017, all of which were examined by multislice spiral CT and MRI, the detection rate of femoral head necrosis and the detection rate of early signs were compared with different methods of examination. Results Avascular necrosis of the femoral head detection rate of MRI was higher than CT, and I~II stage of femoral head necrosis detection rate of MRI was higher than that of CT, there was significant difference (P<0.05); in the patients with I~II stage of femoral head necrosis, the line like sign, bone marrow edema, trabecular fuzzy detection rate of MRI was higher than CT, there was significant difference (P<0.05), and the cystic change detection rate of two methods was not statistically significant (P>0.05). Conclusion MRI is superior to CT in the diagnosis of early osteonecrosis of the femoral head, especially the typical signs of bone marrow edema, linear sign and trabecular bone fuzziness,it has good clinical diagnostic value.
