目的 评估患者治疗教育在不同年龄组儿童特应性皮炎(atopic dermatitis,AD)治疗管理中的作用.方法 对53例1～11岁AD患儿进行过敏原检测和患者治疗教育,并将其分为1～4岁和5～11岁两组.3个月后进行随访,分别在初诊和随访时比较AD严重度评分(SCORAD)、患儿及其家庭生活质量评分(IDQOL/CDLQI,DFI).结果 AD患儿sIgE抗体阳性37例(69.81％),初诊时IDQOL评分(8.94±3.57)分,CDLQI评分(8.00±2.70)分,DFI评分分别为(8.54±4.50)分和(7.17±3.38)分.IDQOL/CDLQI评分最高的前3项是瘙痒、情绪和睡眠,低年龄组DFI评分最高的前3项是睡眠、疲劳和购物花费,高年龄组DFI评分最高的前3项是疲劳、睡眠和情绪,说明AD对两个年龄组家庭生活质量影响存在一定差异.与初诊时比较,两个年龄组AD患儿治疗教育3个月后随访时疾病严重度、患儿及其家庭生活质量均有显著改善,差异有统计学意义(P均为0.000).结论 患者教育在AD患儿长期治疗管理中发挥着重要作用,针对不同年龄患儿,可以进行差异化的患者教育.%Objective To evaluate the effect of therapeutic patient education in the management of atopic dermatitis(AD) in children of different age.Methods A total of 53 children with AD were enrolled,and the patients were divided into two groups,1～4 years old and 5～11 years old.Serum IgE antibodies detection and conventional treatment were given to all the patients,and education associated with AD were carried out to their parents.After three months,the severity of disease and quality of life in children and their families were compared to those at base line.Results The positive rate of specific IgE antibodies was 69.81％.At base line,the mean scores of IDQOL/CDLQI were (8.94±3.57)/(8.00±2.70),and the mean scores of DFI were (8.54±4.50) and(7.17±3.38) for the two groups respectively.The top three scores of IDQOL/CDLQI items were pruritus,emotion and sleep.The top three scores of DFI items for 1～4 years old group were sleep,tiredness and shopping cost of family;and those for 5～11 years old group were tiredness,sleep and emotion of family,the data indicated that AD may cause different influence on the quality of life of the two different age groups.After three months of intervene,both the two groups obtained significant improvements in the severity of disease and quality of life in children and their families(P=0.000).Conclusion The therapeutic patient education plays an important role in the long-term treatment of AD,differential therapeutic patient education may be carried out to the patients of different age.
