Objective To explore the relationship between the Charlson Comorbidity Index (CCI) and the risk of fall among patients with acute coronary syndrome (ACS),and to provide reference for targeted intervention to prevent fall.Methods The demographic data and related clinical indicators of 661 patients with ACS were retrospectively collected,then their risk of fall were assessed using fall risk assessment table,and their complications were assessed using CCI,lastly the relationship between fall of risk and the CCI was analyzed.Results The score of fall risk was (8.39± 3.48),the median score was 8.The fall risk of 435 (65.81％) patients was low,and 226 (34.19％) was high.Logistic regression analysis showed that,women,history of fall,high level of cardiac function and high CCI score were risk factors of fall risk (P＜0.01 for all).CCI score was(3.15±1.80),which was positively correlated with the score of fall risk (P＜0.01).The patients with high risk of fall scored higher in CCI than the CCI score of the others with lower risk of fall (P＜0.01).The area under the ROC curve of CCI predicting fall risk was 0.790 (95％CI,0.753-0.827,P＜0.01),which had good prediction value.Conclusion The influencing factors of fall risk among patients with ACS are multifactorial,clinical nurses could fast recognize the high risk population of fall at an early time using CCI,and give them predictive and individualized intervention.%目的 探讨Charlson合并症指数(CCI)与急性冠脉综合征患者跌倒风险的关系,为开展针对性跌倒预防护理干预提供参考.方法 回顾性收集661例急性冠脉综合征患者人口学资料和相关临床指标,采用跌倒风险评估表进行跌倒风险评分,CCI评估合并症情况,并分析患者跌倒风险与CCI的关系.结果 患者跌倒风险评分为(8.39±3.48)分,中位分值为8分.跌倒风险低者435例(65.81％),高风险者226例(34.19％).Logistic回归分析显示,女性、既往有跌倒史、心功能分级高和CCI得分高是跌倒风险的危险因素(均P＜0.01).CCI得分(3.15±1.80)分,CCI得分和跌倒风险评分呈正相关(P＜0.01).跌倒高风险者CCI得分显著高于跌倒低风险者(P＜0.01).CCI预测跌倒风险的ROC曲线下面积为0.790(95％CI 0.753～0.827,P＜0.01),有较好的预测价值.结论 急性冠脉综合征患者跌倒风险的影响因素是多方面的,护士可结合CCI,早期快速识别跌倒高风险人群,并给予预见性、个性化的护理措施.
