目的 探索慢性心力衰竭患者健康相关生活质量及影响因素.方法 采用描述性研究设计方法,以便利抽样方法抽取3家三级医院心血管内科住院的263例慢性心力衰竭患者,采用自设患者资料调查表、明尼苏达心力衰竭生存质量量表,收集患者一般人口学资料、病情相关资料、生活背景资料及健康相关生活质量状况.进行单因素及多元逐步回归分析.结果 263例慢性心力衰竭患者明尼苏达心力衰竭生存质量量表总分为(60.20±21.86)分.单因素分析显示基础心脏疾病、左心室射血分数、心功能分级、近3月住院次数、是否经常运动、是否热心社会活动、家庭成员人数、性格是否开朗、有无宗教信仰是其总分的影响因素(P<0.05).以明尼苏达心力衰竭生存质量量表得分为因变量、上述9个因素为自变量,经多元逐步回归分析,结果显示心功能分级、是否经常运动、家庭成员人数是影响慢性心力衰竭患者健康相关生活质量的独立因素(P<0.05).结论 心功能较好、经常参加运动、有3人及以上家庭成员的患者健康相关生活质量较好.%Objective To study the health related quality of life and its influencing factors for patients with chronic heart failure.Method Choose 263 cases of inpatients with chronic heart failure from department of cardiovascular internal medicine in 3 top level hospitals by convenient sampling using descriptive study method.Collect patients' information of general population information, disease related information, life background and health related quality of life status by self-designed questionnaire and Minnesota Living with Heart Failure Questionnaire (MLHFQ).Make single factor and multiple stepwise regression analysis.Result The total score on MLHFQ for 263 patients is (60.20±21.86).Single factor analysis shows that basic cardiovascular diseases, left ventricular ejection fraction, cardiac function classification, hospitalization time in 3 months, exercise amount, desire on social activities, family members, extraversion personality and religious belief are influencing factors of total score (P<0.05).Multiple stepwise regression with score on MLHFQ as dependent variable and 9 factors above as independent variables shows that cardiac function classification, exercise amount and how many family members are independent factors of quality of life for patients with chronic heart failure (P<0.05).Conclusion Patients with nice cardiac function, regular exercise and more than 3 family members have great health related quality of life.
