应用六西格玛法提高神经内科出院病人2周复诊率

         

摘要

目的 探讨应用六西格玛方法提高神经内科出院病人2周专科门诊复诊率.方法 应用六西格玛质量管理体系中的定义、测量、分析、改进和控制5个步骤对神经内科病人出院至复诊的流程进行分析,找出影响病人复诊的主要原因,制定相应的改进措施,如增加出院常规宣教项目、发放复诊联系卡、与病人或家属协商确定回访时间及方式等,以提高出院病人的复诊率.结果 神经内科病人复诊率从29.21%提高到41.67%,差异有统计学意义(χ2=10.448,P=0.001).结论 应用六西格玛方法,通过团队合作实施流程改进,是提高出院病人复诊率的有效办法.%Objective To explore the improvement of return visit for discharged patients within two weeks with Six Sigma in Neurology department. Methods Five steps in Six Sigma including def'inition, assessment, analysis, improvement and control were applied to analyze the process from patients' discharge to return visit and work out corresponding improving methods such as increasing health education, sending return visit card. Results The percentage of return visit was improved from 29.21% to 41.67%,which was with statistical significance (X2=10.448, P=O.OO1). Conclusion With the application of Six Sigma for process improvement,the percentage of return visit of discharged patients was improved effec:ively.
