患者进食后接受结肠镜检查最佳时间的探讨

             

摘要

目的探讨患者进食面条、点心、普食后接受结肠镜检查的最佳时间。方法将结肠镜检查患者随机分为点心组、面条组、普食组,每组清洁肠道后在检查前进食相应的食物,每组再随机分为4个亚组,每亚组100例,4个亚组在进食后不同时段接受结肠镜检查,对比每组的4个亚组的肠道清洁程度、肠道清洁范围、对检查的耐受性、进食后不良反应;观察100例未清洁肠道的消化道钡餐检查患者钡剂到达回盲部的时间;将12例志愿者随机分为点心组、面条组、普食组,每组在肠道清洁后进食相应的食物加钡剂,观察每组钡剂到达回盲部的时间。结果点心组、面条组及普食组中前3个亚组的肠道清洁程度及肠道清洁范围比较,差异均无统计学意义(P>0.05),点心组、面条组及普食组的4个亚组的肠道清洁程度及肠道清洁范围比较,差异均有统计学意义(P<0.05),点四组、面四组、普四组的肠道清洁程度及肠道清洁范围明显差于同一组中的其他小组,即进点心后>2~2.5 h、进面条后>2~2.5 h、进普食后>2.5~3 h进行结肠镜检查,患者的肠道清洁受到影响。点心组、面条组及普食组中4个亚组患者对检查的耐受性差异无统计学意义。面一组、普一组和普四组各有1例患者发生Ⅱ级不良反应(恶心),点心组无1例出现恶心,所有组均没有发生呕吐胃内容物的情况。未清洁肠道的100例消化道钡餐检查患者空腹进钡餐后,钡剂到达回盲部的时间为(4.06±1.13)h。清洁肠道后进食食物+消化道钡餐的3组志愿者,钡剂到达回盲部的时间:点心组2.17~2.58 h,平均2.42 h;面条组2.17~2.67 h,平均2.44 h;普食组2.58~3.41 h,平均2.89 h。结论进食点心或面条后0.5~2 h、进食普食后1~2.5 h是患者接受结肠镜检查的最佳时间。%Objective To study the optimal time for colonoscopy after eating noodles, snacks and common food. Methods The patients undergoing colonoscopy were randomly divided into noodle group, snack group and common food group. Patients in each group had the corresponding foods after intestinal cleaning then each group was randomly divided into 4 teams with 100 cases per team. Patients in each team underwent colonoscopy in different periods. Intestinal cleaning degree, cleaning range, tolerance, adverse reactions after eating were compared. Meanwhile, 100 patients without intestinal cleaning underwent gastrointestinal barium meal examination were observed to find out the time of barium arriving at the ileocecal. Twelve volunteers were randomly divided into snack group, noodle group, and common food group then had corresponding foods plus barium after intestinal cleaning. Results The comparisons of intestinal cleaning degree and cleaning range of the first three teams in snack group, noodle group and common food group showed no significant difference (P>0.05), but the difference of intestinal cleaning degree and cleaning range of the 4th team was statistically significant (P<0.05). The intestinal cleaning of the patients undergoing colonoscopy 0.5~2 h after having snack, noodle, or 1~2.5 h after having common food was not influenced. The comparison of inspection tolerance of 4 teams in each group was not significant different (P>0.05). Only 3 cases occurred adverse reaction in the inspection process. The time of gastrointestinal barium meal examination with barium reaching ileocecal was (4.06 ±1.13)h. Conclusion The optimal time for colonoscopy after eating is that 0.5~2 h for snack or noodle and 1~2.5 h for common food.
