基于患者结局的PICC置管术后护理敏感性结局指标的构建

摘要

目的 构建PICC置管患者术后的护理敏感性结局指标.方法 以奥马哈问题分类系统为研究框架,护理结局分类系统为理论基础,采用Delphi法对26名资深专家完成三轮有效函询,获得专家一致性意见,确立出PICC置管患者术后的护理敏感性结局指标.结果 3轮函询问卷有效回收率分别为93%、96%、96%,专家权威系数分别为0.936、0.941、0.939,一、二、三级指标协调系数分别为0.341、0.382、0.412,具有统计学意义(P<0.01),最终构建的PICC置管患者术后的护理敏感性结局指标体系包含4个领域、7个一级指标和14个二级指标,42个三级指标,各级指标选择意见逐步趋同.结论 建立的PICC置管患者护理敏感性结局指标体系包括7个一级指标、14个二级指标、42个三级指标,经检验,结果可靠,信度较高,可为护理人员制定有针对性的延续护理模式提供理论依据.%Objective To develop nursing sensitive outcome index for patients with PICC. Methods Based on Omaha System and NOC and with the method of Delphi, 3 rounds of consultation among 26 experts were completed and after consensus was reached, the nursing sensitive outcome index for patients with PICC was developed. Results The effective response rate of 3 rounds of expert consultation was 93%, 96%and 96%respectively and the authority coefficient was 0.936, 0.941 and 0.939 respectively. The coordination coefficient of primary, secondary and third indicators was 0.341, 0.382 and 0.412 respectively ( P<0.01). The nursing sensitive outcome index for patients with PICC consisted of 4 domains, 7 first-1evel indicators, 14 second-level indicators and 42 third-1evel indicator. Conclusion The established nursing sensitive outcome index for patients with PICC includes 7 primary indicators, 14 secondary indicators and 42 three indicators. With high reliability and validity, it provides reference for the development of targeted extended nursing care model.
