HIF-1α在乳腺癌合并2型糖尿病患者中的表达

         

摘要

目的:探讨乳腺癌合并2型糖尿病中缺氧诱导因子-α(HIF-1α)的表达及其对血管内皮生长因子(VEGF)、微血管密度(MVD)的影响.方法:回顾性分析我院2011年1月-2014年12月收治的118例乳腺癌合并2型糖尿病患者(实验组)和非糖尿病的乳腺癌患者的临床资料(对照组),比较两组基本临床病理特征差异,采用免疫组化SP法检测两组患者HIF-1α及VEGF的表达,采用CD34对血管内皮细胞进行标记并计算MVD.结果:实验组中肿瘤直径大于2cm的人数远多于对照组,差异有统计学意义(χ2=4.434,P=0.035);对照组和实验组淋巴结转移率分别为43.75%(56/128)、59.32%(70/118),实验组的淋巴结转移率远高于对照组(χ2=5.959,P=0.015);实验组的HIF-1α阳性表达率为80.51%,显著高于对照组的HIF-1α阳性表达率(64.84%),差异存高度统计学意义(χ2=7.533,P=0.006),实验组的VEGF阳性表达率为86.44%,高于对照组的VEGF阳性表达率(68.75%),差异存高度统计学意义(χ2=10.929,P=0.001);实验组的MVD计数高于对照组,差异存高度统计学意义(t=2.883,P=0.004).实验组中HIF-1α阳性表达组VEGF阳性表达率为90.53%(86/95),HIF-1α阴性表达组的VEGF阳性表达率为69.57%(16/23),HIF-1α阳性表达组明显高于HIF-1α阴性表达组,两组比较差异有统计学意义(χ2=6.941,P=0.008),两组的MVD计数分别为121.35±32.46,102.74±34.86,差异存统计学意义(t=2.432,P=0.017).结论:2型糖尿病能促进乳腺癌组织微血管的增生,加速肿瘤组织的转移与增长,是乳腺癌患者预后差及临床分期晚的危险因素.%Objective:To explore the expression of hypoxia-inducible factor 1 alpha(HIF-1α)in tissue of breast cancer complicated with diabetes and examine the correlation with vascular endothelial growth factor(VEGF)and microvascular density(MVD)and correlation with vascular endothelial growth factor(VEGF)and microvascular density(MVD).Methods:The clinical data of 118 cases of breast cancer combined with type 2 diabetes(experimental group)and breast cancer patients without diabetes(control group)were analyzed retrospectively from January 2011-December 2014. Immunohistochemical Streptavidin-Perosidase(SP)method was used to detect the expressions of HIF-1α and VEGF in breast cancer tissue of two groups.Vascular endothelial cells were tagged with CD34 to calculate MVD.Results:Tumor diameter greater than 2cm in the experimental group was more than the control group,with statistical significance(χ2=4.434,P=0.035).The positive rate of the control group and experimental group lymph node metastasis rate was 43.75%(56/128),59.32%(70/118),lymph node metastasis rate of the experimental group was much higher than the control group(χ2=5.959,P=0.015).HIF-1α positive expression rate of experiental group was 80.51%,significantly higher than the control group of HIF-1α positive expression rate(64.84%),and there were difference highly statistical significance(χ2=7.533,P=0.006).VEGF positive expression rate of the experimental group was 86.44% higher than that of control group VEGF positive expression rate(68.75%),with statistical significance(χ2=10.929,P=0.001).Experimental MVD count was higher than the control group,with high statistical significance(t=2.883,P=0.004).HIF-1α positive expression in the experimental group of VEGF positive expression rate was 90.53%(86/95),HIF-1α negative expression group was 69.57%(16/23),the positive expression of HIF-1α group was obviously higher than that of HIF-1 alpha negative expression,were with statistical significance(χ2=6.941,P=0.008),the MVD counting of the two groups were 121.35±32.46,102.74±34.86,with statistically significant (t=2.432,P=0.017).Conclusion:Type 2 diabetes can promote the proliferation of breast cancer tissue microvascular,accelerate the transfer of tumor tissue and growth,is the risk factors of breast cancer with poor prognosis and late clinical stage.
