帕罗西汀治疗慢性心力衰竭合并抑郁症患者的疗效观察

摘要

目的：探讨帕罗西汀对慢性心力衰竭合并抑郁症患者的临床治疗疗效。方法将2010年1月至2012年6月52例伴抑郁症的慢性心力衰竭患者随机分为对照组和治疗组各26例，所有患者均给予常规抗心力衰竭治疗，治疗组在此基础上加用帕罗西汀抗抑郁治疗，疗程12周。分别于治疗前后测量和记录患者的左心室射血分数（LVEF）、生活质量（QOL）以及汉密尔顿抑郁量表-17（HAMD-17）评分，并计算抗抑郁治疗有效率。结果治疗组治疗后LVEF较对照组有明显提高，差异有统计学意义（P<0.05），QOL评分以及HAMD量表评分均有明显下降，差异均有统计学意义（P<0.05）。治疗组抗抑郁治疗总有效率达69.2%（18/26），对照组为38.5%（10/26），两组比较，差异有统计学意义（P<0.05）。结论帕罗西汀可以改善慢性心力衰竭合并抑郁症患者的抑郁症状，改善心功能，提高患者的QOL，值得临床推广。%Objective To explore the clinical effects of paroxetine in the treatment of patients with chronic heart failure (CHF) complicated with depression. Methods A total of 52 patients with CHF complicated with depression,who were hospital-ized in the department from January 2010 to June 2012 ,were randomly divided into control group and treatment group ,26 cases in each group. All the patients received conventional treatment for heart failure ,and the treatment group was added with paroxetine for anti-depression,12 weeks were regarded as a period. To determine the left ventricular ejection fraction (LVEF),the quality of life (QOL) and the Hamilton Depression Scale(HAMD) scores before and after treatment,and to calculate the effective rate of antide-pressive therapy. Results The LVEF in the treatment group improved more obviously than that in the control group (P<0.05);the QOL score and HAMD score in the treatment group decreased more significantly than those in the control group (P<0.05);the total effective rate of antidepressive therapy was 69.2%(18/26) in the treatment group and 38.5%(10/26) in the control group,and the difference between the two groups had statistically significant difference (P<0.05). Conclusion Paroxetine can improve de-pressive symptom of patients with chronic heart failure complicated with depression,increase cardiac function and enhance QOL, so it is worthy popularizing clinically.
