骨瓜提取物注射液治疗膝骨性关节炎疗效观察

摘要

Objective :To observe the clinical efficacy and potential therapeutic mechanism of Cervus and Cucumis Poly-peptide for Injection on the Osteoarthritis of knee .Methods :60 patients were randomly divided into 2 groups .Cervus and Cucumis Polypeptide for Injection 50mg that added in 250ml of 0 .9% sodium chloride injection was used in treat-ment group ,1 time a day ;glucosamine capsules 480mg each time ,was taken in control group ,3 times a day .At the same time Nimesulide Capsules 100mg each time ,was used in bouth treatment group and control group ,2 times a day . 30 days later ,curative effect to be evaluated ,and IL-1 level were detected in knee joint fluid interleukin in order to ob-serve the changes .During the time other anti-inflammatory analgesic ,adrenal glucocorticoids and other relieve symp-toms drugs for the treatment of OA slow action .were banned .Results:The total effectiveness of treatment group is bet-ter than the control group ,and there is a statistical difference(χ2 =5 .455 ;P=0 .042<0 .05);the IL-1 value in treat-ment group and control group were significantly decreased than before treatment (T= 3 .74 ,4 .01 ;P< 0 .05) .The difference of IL-1 between the two groups after treatment ,there are significant difference (T=5 .52 ,P< 0 .05) .Con-clusion:Cervus and Cucumis Polypeptide for Injection has a good curative effection on knee osteoarthritis ;and it can re-duce IL-1 value which in the knee joint fluid abnormally elevated by knee Osteoarthritis .%目的：观察骨瓜提取物注射液治疗膝骨性关节炎的临床疗效及其潜在治疗机制。方法：将60例患者随机分为2组，试验组30例用骨瓜提取物注射液50mg加入250ml 0．9％氯化钠注射液中静脉滴注，1次/d ，同时口服尼美舒利胶囊，100mg/次，2次/d；对照组30例口服氨基葡萄糖胶囊，480mg/次，3次/d ，同时口服尼美舒利胶囊，100mg/次，2次/d。连续治疗30d后评定疗效，并检测治疗前、后膝关节液中白细胞介素-1（IL-1）的水平变化。期间停服其他抗炎镇痛药、肾上腺糖皮质激素和其他缓解症状的治疗OA慢作用的药物。结果：试验组总有效率优于对照组，且存在统计学差异（χ2＝5．455；P＝0．042＜0．05）；试验组和对照组患者膝关节液IL-1值与治疗前比较均明显下降，且有统计学意义（ T＝3．74，4．01；P均＜0．05）；两组治疗后IL-1差值比较，有显著性差异（ T＝5．52；P＜0．05）。结论：骨瓜提取物注射液能降低膝骨性关节炎病患者膝关节液中IL-1的异常升高，对治疗膝骨性关节炎有较好疗效。
