目的：总结胰腺囊性肿瘤的临床特点及诊治方法。方法：回顾性分析内蒙古边防总队医院肝胆胰外科2011年1月－2014年6月收治并经病理确诊的78例胰腺囊性肿瘤患者的临床资料，包括发病年龄、临床症状、手术方式、术后并发症等。结果：78例患者中，浆液性囊性肿瘤（SCN ）为31例，占39．74％；黏液性囊性肿瘤（MCN ）为40例，占51．28％；导管内乳头状黏液性囊腺瘤（IPM N ）为5例，占6．41％；实性假乳头状瘤（SPN ）为2例，占2．56％。临床症状主要表现为腹部不适、间断性腹痛，少数患者见黄疸、恶心、呕吐等症状。多因素分析结果表明高龄和CA19‐9、CA242水平升高是肿瘤恶性的独立危险因素。术后并发症发生率为21．79％，主要为胰瘘。结论：胰腺囊性肿瘤多发生于中年女性，生长部位以胰体尾部和胰头部多见，病理类型以黏液性囊性肿瘤最为多见；肿瘤标志物检查结果可能对黏液性囊性肿瘤术前的良恶性判断有一定的指导意义；该疾病手术切除率高，胰瘘为常见术后并发症，预后良好。%Objective :To summary the experience of diagnosis and surgical treatment of 78 patients with pancreatic cystic neoplasm .Methods :This study were retrospectively analyzed the data of 78 patients with pancreatic cystic neo‐plasm in the general surgery department of the armed police Border Defense Corps Hospital of Inner Mongol from Janu‐ary 2011 to June 2014 ,analysis the data of pancreatic cystic tumor in age ,clinical symptoms ,operation mode and post‐operative complications .Results:Of the 78 patients ,serous cystic neoplasm (SCN) in 31 cases ,accounted for 39 .74% ;mucinous cystic neoplasm (MCN) of 40 cases ,accounted for 51 .28% ;intraductal papillary mucinous cystic adenoma (IPMN) for 5 cases ,accounting for 6 .41% ;solid false papilloma (SPN) in 2 cases ,accounting for 2 .56% .Clinical symptoms were mainly about abdominal discomfort ,intermittent abdominal pain ,a few patients with jaundice ,nausea , vomiting and other symptoms .Multi‐factor analysis results showed that old age and CA19‐9 ,the CA242 levels are in‐dependent risk factors for the development of malignant tumor .Postoperative complications were mainly about pancre‐atic fistula ,the incidence of 21 .79% .Conclusion:PCNs were common in the middle‐aged women ,location in the pan‐creatic body and tail of the pancreatic head and were MCN .Tumor marker examination results may have some guiding significance for judgement on benign and malignant tumor before operation .The disease operation resection rate was high ,pancreatic fistula was a common postoperative complications ,the prognosis was good .
