内外踝联合手术治疗双踝骨折的疗效分析

摘要

Objective :To evaluate the clinical outcome of combined medial and lateral malleolus surgical treatment for bimalleolar fractures .Methods :38 patients suffered from the bimalleolar fracture were treated with internal fixation via combined medial and lateral malleolus approach from May 2008 to May 2013 ,the data were analyzed retrospectively . All patients were evaluated with the healing of fracture ,instability ,walking ability ,radiological manifestations ,and the Baird‐Jackson score .Results:All fractures patients had bone union ,time to bone union varied from 10~14 weeks with an average of 12 .7 weeks .The Baird‐Jackson score was(93 .8 ± 7 .3) ,suggesting an excellent result .Conclusion:Combined medial and lateral malleolus surgical treatment for bimalleolar fractures allows direct reduction and fixation of the bone fracture fragments ,ensure anatomical joint restoration ,enable early functional exercise ,and avoid the oc‐currence of traumatic arthritis ,is an effective treatment for bimalleolar fractures .%目的：探讨内外踝联合手术治疗双踝骨折的临床疗效。方法：回顾性分析2008年5月－2013年5月期间因双踝骨折行手术治疗且有完整随访资料者38例，手术均采用外侧联合内侧入路切开复位钢板螺钉内固定；分析患者踝关节骨折愈合情况、内固定系统稳定性和踝关节Baird‐Jackson评分。结果：所有患者均获得骨性愈合，愈合时间10～14周，平均12．7周；远期踝关节功能Baird‐Jackson评分（93．8±7．3）分，优良率84．2％。结论：内外踝联合手术治疗双踝骨折，术中可以使得踝关节骨折获得解剖复位、坚强内固定，为早期踝关节功能锻炼提供了坚实保障，有效预防创伤性关节炎等并发症，是治疗双踝骨折的可靠手段。
