Objective :To study the clinical efficacy of adjuvant therapy with glucocorticoids in children with lobar pneumonia of sepsis .Methods :Thirty‐seven children with lobar pneumonia of sepsis were randomly divided into treat‐ment group and control group .Both groups were treated with antibiotics of beta‐lactam antibiotics associated with mac‐rolide antibiotics according to pneumonia guidelines .In addition to the basic treatment ,treatment group was given meth‐ylprednisolone 1~2mg/(kg · d) by intravenous drip until the body temperature was normal .Then gave oral prednisone tablets 0 .5~1mg/(kg · d) (gradually reduced) for a total treatment course of 5~7 days .Before and after treatment serum C‐reactive protein (CRP) ,erythrocyte sedimentation rate (ESR) ,and procalcitonin (PCT )were measured .Re‐sults:The duration of fever ,cough relief time ,pulmonary rales ,hospitalization and pulmonary shadow absorption time on chest X‐ray were significantly shorter in the treatment group than in the control group (P<0 .05) .After treatment , the two groups showed improvements in serum CRP ,PCT and ESR(P<0 .05) ,but the treatment group showed sig‐nificantly more improvement(P<0 .05) .Conclusion:Adjuvant therapy with glucocorticoids can effectively alleviate clin‐ical symptoms ,promote the absorption of pulmonary inflammation ,and shorten hospital stay or reduce the cost of hos‐pitalization in children with lobar pneumonia of sepsis .%目的：探讨小剂量糖皮质激素辅治小儿大叶性肺炎伴脓毒症的临床疗效。方法：将37例大叶性肺炎伴脓毒症患儿随机分为治疗组19例和对照组18例。所有患儿据儿童肺炎指南治疗（β内酰胺抗生素或青霉素类联合大环内酯类），治疗组在此基础上予甲泼尼龙琥珀酸钠针剂，剂量1～2mg／（kg · d），待体温正常后改为泼尼松口服序贯治疗，剂量为0．5～1mg／（kg · d），逐渐减量，总疗程5～7d。两组患儿治疗前、后进行CRP、PCT、ESR炎性指标的测定。结果：治疗组显效率和总有效率均高于对照组，差异有统计学意义（P＜0．01）。治疗组热退时间、咳嗽明显减轻时间、肺部啰音减轻时间、肺部阴影吸收时间及住院时间均比对照组明显缩短，差异有统计学意义（P＜0．05）。两组患儿血清CRP、PCT、ESR炎性指标均有明显改善（P＜0．05），但治疗组较对照组改善更显著（P＜0．05）。结论：小剂量糖皮质激素辅助治疗小儿大叶性肺炎伴脓毒症可提高疗效，明显改善临床症状，促进肺部炎症吸收，缩短住院时间及降低住院费用。
