床边吞咽功能评价工具对脑卒中合并吞咽障碍患者并发症及康复的影响

摘要

目的：探讨床边吞咽功能评价工具（GUSS）对脑卒中合并吞咽障碍患者并发症及康复的影响。方法：2014年10月－2015年10月选取本院收治的120例脑卒中合并吞咽障碍患者为研究对象，根据随机数字表将患者分为观察组及对照组，对照组采用洼田试验法进行评估，观察组应用GUSS进行评估，对比分析两组患者并发症及预后情况。结果：对照组吸入性肺炎、误吸、窒息、代谢紊乱、营养不良发生率显著高于观察组（P＜0．05）。观察组入院第2周FMA评分、Barthel指数及SIS评分显著高于对照组（P＜0．05），而NIHSS评分显著低于对照组（P＜0．05）。结论：应用GUSS能及时对脑卒中患者吞咽功能进行评估，并能尽早对患者饮食安全性进行指导，降低患者误吸、吸入性肺炎及窒息等并发症发生，改善患者营养状况，并对患者康复起到一定的促进作用。%Objective :To investigate the bedside swallowing evaluation tool (GUSS) merger of stroke patients with dysphagia complications and rehabilitation .Methods :120 cases of stroke patients with dysphagia merger were divided into observation group(n=60)and control group(n=60) from October 2014 to October 2015 .The control group were assessed with the test method Kubota ,observation group were assessed with GUSS assessment .The complications and prognosis of two groups were analyzed .Results:The aspiration pneumonia ,aspiration ,suffocation ,metabolic disorders , malnutrition of observation group were higher than in the observation group (P<0 .05) .The FMA score ,Barthel Index and SIS scores of observation group were higher than observation group (P<0 .05) and NIHSS scores were significant‐ly lower than the control group (P<0 .05) .Conclusion:GUSS can promptly assess swallowing function in stroke pa‐tients ,and patients as early as possible for the safety of dietary guidance ,reduce patient aspiration ,aspiration pneumo‐nia and complications such as asphyxia ,improve the nutritional status of patients ,and rehabilitation of patients play a role in promoting .
