Objective :Presepsin ,also called soluble CD subtype ,as a new type of sepsis biomarkers displays great value in sepsis diagnosis .However ,there were few reports about this biomarker in China .Therefore ,we conducted a prospec-tive cohort study to assess the diagnostic accuracy of Presepsin .Methods :We collected the serum samples taken from patients with Systemic Inflammatory Response Syndrome (SIRS) and evaluated the diagnostic accuracy of PCT ,CRP and Presepsin .Results:A total of 191 patients were included in our study ,including 84 non-infectious SIRS patients and 107 sepsis patients .The area under the receiver operating characteristic curve was 0 .90 ,0 .85 ,0 .76 for Presepsin , PCT and CRP respectively .There is no significant difference between Presepsin and PCT in sepsis diagnosis .Conclu-sion:Presepsin as a novel biomarker showed good clinical value for differential diagnosis between sepsis and non-infec-tious SIRS .However ,the difference between Gram-positive and Gram-negative bacteria and the value for guiding anti-biotic therapy needed studies to further confirm .%目的：Presepsin（可溶性CD14亚型）为一种新型脓毒症诊断标志物，展现出较好的诊断价值，但在国内报道较少，因此，笔者前瞻性纳入全身炎症反应综合征（SIRS）患者，评价Presepsin对脓毒症的诊断价值。方法：前瞻性纳入满足SIRS患者，收集患者血清，评价患者C反应蛋白（CRP）、降钙素原（PCT ）、Presepsin等标志物对脓毒症的诊断价值。结果：研究一共纳入191例患者，包括84例非感染性SIRS患者与107例脓毒症患者。三种标志物的ROC曲线下面积分别为0．90（Presepsin）、0．85（PCT）、0．76（CRP）。Presepsin与 PCT 两者之间诊断价值无明显差异。结论：作为一种新型标志物，Presepsin展现出较好的诊断价值，但其对革兰氏阳性菌与阴性菌的鉴别价值与对抗生素的指导作用有待进一步证实。
