多层螺旋CTA评价Willis环变异及其与脑血管病的关系

         

摘要

Objective: To inquire into anatomic variation of Willis circle and its relation with cerebrovascular disease. Methods:The 16-slice spiral CT angiography (CTA) was performed on 76 patients, who were diagnosed with various cerebrovascular disease in clinical diagnosis. We used multiple ways to rebuilt willis circle and observed it from various perspectives. Results: Among those 76 patients, the major anatomic variations of willis circle were as follows: anterior cerebral artery (ACA) single branch with multiple trunk, fetal origin of the posterior cerebral artery, ACA Al aplasia. Fetal origin of the posterior cerebral artery with the other side of ACA Al aplasia or absence (ACA single branch with multiple trunk). Third branch ACA. Etc. Among those who had cerebral aneurysms. Anatomic variation of willis circle in cerebral aneurysms was 83%(38/46). Among those cerebral infarctions, anatomic variation of willis circle in cerebral infarctions was 53% (19/36). Conclusion: The anatomic variation of willis circle was comparatively great, and it has close relation with the occurrence of cerebrovascular disease and the hemodynamic changes.%目的:探讨Willis环的解剖变异及其与脑血管病的关系.方法:对76例临床诊断为各种脑血管疾病患者行16层螺旋CT脑血管成像,行多种方式重组、多方位观察Willis环.结果:76例患者中.Willis环主要解剖变异有:大脑前动脉单支多于.胚胎型大脑后动脉,大脑前动脉的交通前段发育不良,胚胎型大脑后动脉并对侧大脑前动脉的交通前段发育不良或缺如(单支多干),第三支大脑前动脉等.动脉瘤患者中Willis环解剖变异者约为83% (38/46),脑梗死患者中Willis环解剖变异者约为53%(19/36).结论:Willis环的解剖变异较大,且与脑血管病的发生以及血流动力学改变关系密切.
