数字摄影中患者辐射剂量和曝光指数调查研究

         

摘要

Objective To investigate the patient radiation dose and exposure index in digital radiography .Methods The analysis consisted of measurement of the dose‐area product (DAP) and entrance surface dose (ESD) received by a reference group of patients for 8 common radiographic procedures using DR system .Meanwhile ,exposure index (EI) data were col‐lected and compared with previous study .Results The average patient ESD for each radiographic procedure was 0 .29 mGy (chest PA) ,0 .27 mGy (chest LAT) ,1 .41 mGy (skull PA) ,0 .95 mGy (skull LAT) ,2 .12 mGy (lumbar spine AP) ,4 .60 mGy (lumbar spine LAT ) ,1 .42 mGy (abdomen AP) ,and 1 .36 mGy (pelvis AP) ,respectively .The average patient EI for each radiographic procedure was 1803 (chest PA ) ,1562 (chest LAT ) ,1719 (skull PA ) ,1762 (skull LAT) ,1871 (lumbar spine AP) ,1654 (lumbar spine LAT) ,1982 (abdomen AP) ,and 1788 (pelvis AP) ,respectively . The average EI was 1768 for images ,which qualified the EC standard .Conclusion The patient doses and EI are higher than necessary .Optimization procedure should be performed to adjust radiographic parameters .%目的:调查分析数字摄影中患者辐射剂量和曝光指数(exposure index ,EI),探讨EI对于数字摄影中辐射防护最优化的意义。方法在应用(digital radiography ,DR)系统摄影中选取100名接近标准体重和躯干前后径的患者作为参考人群,8个常用摄影体位作为测量对象,测量每次曝光患者剂量‐面积乘积(dose‐area product ,DAP)和体表入射剂量(entrance surface dose ,ESD)。同时记录曝光后的 EI值。评价合格影像EI水平。结果每个摄影体位的患者平均ESD分别为:胸部后前位(0.29),胸部侧位(0.27),头颅后前位(1.41),头颅侧位(0.95),腰椎前后位(2.12),腰椎侧位(4.60),腹部前后位(1.42),骨盆正位(1.36)。每个摄影体位的患者平均 EI值分别为:胸部后前位(1803),胸部侧位(1562),头颅后前位(1719),头颅侧位(1762),腰椎前后位(1871),腰椎侧位(1654),腹部前后位(1982),骨盆正位(1788)。影像质量能够达到EC标准时的所有影像的平均EI值为1768。结论数字摄影中患者辐射剂量及曝光指数偏高,应适当下调以实现辐射防护的最优化。
