MRI平扫对膝关节髌内侧滑膜皱襞综合征的诊断价值

Objective To evaluate the value of MR imaging on mediopatellar plica syndrome ( MPS) of knee joint.Methods The non-enhanced MR imaging and MRI arthrography of eighty-four knees joint underwent arthroscopy were analyzed retrospective-ly.The diagnostic criteria of MPS of MRI and arthrography were confirmed respectively.The existence and type ( if existed) of MPS were evaluated on non-enhanced MR imaging and MRI arthrography imaging, and compared with those of arthroscopy.Re-sults In 84 knees, MPS was found in 19 ones in arthroscopy with 18 (14 type C, 4 type D) in non-enhanced MR imaging and 21 (16 type C, 5 type D) in MRI arthrography.The sensitivity and specificity were high in both non-enhanced MR imaging and MRI arthrography (94.4%and 98.5%, 100%and 96.9%;respectively).Non-enhanced MR imaging and MRI arthrography had good agreement with arthroscopy (kappa>0.75).Conclusion Non-enhanced MR imaging had great value in the diagnosis and classification of MPS.MRI arthrography can be replaced by non-enhanced MR imaging in the diagnosis of MPS to a certain extant.%目的：评估MRI平扫对膝关节髌内侧滑膜皱襞综合征（ mediopatellar plica syndrome， MPS）的诊断价值。方法回顾性分析84例行关节镜检查的膝关节MRI平扫及MR关节造影资料，分别统一MPS的磁共振及关节镜诊断标准，评估MRI平扫及MR关节造影图像中MPS的存在与否与分型，并与关节镜结果进行对比。结果84例膝关节中，关节镜发现有19例存在MPS，MRI平扫发现18例（ C型14例，D型4例），MR关节造影发现21例（ C型16例，D型5例）。MRI平扫诊断MPS的敏感度（94．4％）和特异度（98．5％）较高，MR关节腔造影诊断MPS的敏感度（100％）和特异度（96．9％）亦较高。 MRI平扫和MR关节造影与关节镜结果的一致性较好（ kappa＞0．75）。结论 MRI平扫对MPS的诊断及分型均具有重要价值，在诊断MPS时MRI平扫在一定程度上可以替代MR关节造影。
