Objective Via analyzing the type of circle of willis by 3D-TOF MRA,to investigate clinical significance of collateral circulation of circle of willis in patients with interal carotid artery occlusion.Methods 1439 patients were subjected to 3D-TOF MRA on head and,meanwhile,underwent the examination of MR plain scan and diffusion imaging.Of those patients,845 were male and 594 were female,aged from 23 to 89 years old,with an average of (57.1 ± 9.5) years.The original images of 3D-TOF MRA were reconstructed.Results In this work,type] (circle of willis) accounted for 47.46％ (683 cases),type Ⅱ accounted for 38.71％ (557 cases),type Ⅲ accounted for 6.25％ (90 cases),and type Ⅳ accounted for 7.57％ (109 cases).There were 60 cases of intracal interal carotid artery occlusion,30 cases were type Ⅰ (circle of willis),21 cases were type Ⅱ,3 cases were type Ⅲ,6 cases were type Ⅳ.53 cases were acute stage infarction,among which 16 presented with a large area infarction,22 with watershed infarction,15 with small focus infarction,7 with negative acute stage lesion,and 2 with piamater arterial anastomosis.In a large area infarction cases,5 cases were type Ⅰ (circle of willis),6 cases were type Ⅱ,0 was type Ⅲ,and 5 were type Ⅳ.Conclusion 3D-TOF MRA can provide reference for the relation of clinical evaluation of circle of willis type and the change of collateral circulation of circle of willis in interal carotid artery occlusion with cerebral infarction degree.%目的 通过对3D-TOF MRA显示willis环的类型分析,探讨单侧颈内动脉闭塞病例中willis环侧枝循环的临床意义.方法 头部3D-TOF MRA检查的1439例患者,并同时行MR平扫及弥散成像检查,男性845例,女性594例,年龄23～89岁,平均年龄(57.1±9.5)岁.对3D-TOF MRA原始图像数据进行最大密度投影重建显示.结果 Willis环Ⅰ型47.46％(683例),Ⅱ型38.71％(557例),Ⅲ型6.25％(90例),Ⅳ型7.57％(109例);单侧颈内动脉颅内段闭塞60例,willis环Ⅰ型30例,Ⅱ型21例,Ⅲ型3例,Ⅳ型6例,急性期梗塞53例,其中大面积梗塞16例、分水岭梗塞22例、小灶梗塞15例,非急性期梗塞病灶7例,软脑膜动脉吻合2例;大面积梗塞病例中willis环Ⅰ型5例,Ⅱ型6例,Ⅲ型0例,Ⅳ型5例.结论 3D-TOF MRA为临床评价willis环的分型及单侧颈内动脉闭塞后脑梗塞程度与willis环侧枝循环之间的关系提供了参考.
