联合磁敏感序列及PET/CT对老年轻度认知障碍及阿尔茨海默病患者海马区的临床研究

     

摘要

Objective To explore the changes of the nerve cell glucose uptake in the hippocampus based on the abnormal ve -nous drainage among the normal elderly people , mild cognitive impairment ( MCI) and Alzheimer's disease ( AD) , from the aspect of venous to study its function of nerve cells .Methods We used subsequence of SWI and PET/CT to scan 15 normal elderly people, MCI and AD patients respectively and detected the Variation characteristics of the bilateral hippocampus draining veins and 18 F-FDG SUVmax changes in the bilateral hippocampus .Results Compared with the normal elderly people , the maximum diameter of left inferior ventricular vein is reduced significantly in MCI , the length and branch numbers are increased , but the SU-Vmax in the left hippocampus has no significant change;the maximum diameter , length, branch numbers and the SUVmax in off-side have no significant changes .Compared with the normal elderly people , the maximum diameter , length and branch numbers in bilateral inferior ventricular veins are reduced significantly in AD , and the SUVmax in the bilateral hippocampus is reduced signif-icantly.Conclusion The changes of the hippocampal draining veins form may lead to the disorder of glucose utilization in hipp -ocampal nerve cells , and there is a certain regular in the process from MCI to AD .%目的 研究正常老年人、老年轻度认知障碍患者(MCI)及阿尔茨海默病患者(AD)海马区引流静脉-侧脑室下静脉变化差异所致的海马神经细胞葡萄糖摄取能力变化,从血管层面对其神经细胞功能进行观察研究.方法 采用高场MRI磁敏感序列(SWI)与PET/CT设备分别对15例正常老年人、15例老年MIC患者及15例AD患者进行头颅扫描检查,检测海马引流静脉-侧脑室下静脉的形态改变特征与海马区18 F-FDG SUVmax值变化规律.结果 与正常老年组对比,老年MCI患者左侧脑室下静脉出现直径变窄、长度变长及分支增多的表现,差异有统计学意义,而左侧海马区SUV-max值未出现有统计学意义的变化,右侧脑室下静脉直径、长度、分支数目及右侧海马区SUVmax值均未出现有统计学意义的变化;与正常老年组对比,AD患者双侧脑室下静脉均出现直径变窄、长度变短及分支减少,差异有统计学意义,双侧海马区SUVmax值均出现有统计学意义的减低.结论 海马引流静脉-侧脑室下静脉的形态变化能够导致海马神经细胞对葡萄糖利用的障碍,并且在老年MCI到AD发展的过程中有一定规律.
