Objective To investigate multi-slice spiral CT (MSCT), gastrointestinal imaging and endoscopic characteristics in primary gastrointestinal lymphoma (PGIL), further to provide reliable basis for clinical diagnosis.Methods Twenty-five patients with PGIL were retrospectively analyzed in the study, including the MSCT data, gastrointestinal imaging and fiber endoscopy.Results CT manifestation: Of 14 cases with primary gastric lymphoma, 10 cases showed diffusely thickened gastric wall, 2 cases had locally thickened gastric wall with nodular or polypoid change in which superficial ulcer could be seen.And all 11 cases with primary intestinal lymphoma appeared diffused or localized thickening.Gastrointestinal imaging finding: Among 11 cases with primary gastric lymphoma, diffuse multiple small nodular or mass-like filling defect was seen in 5, varying degrees of flattened, widened disappearance or destruction in mucosal folds were seen in 2.Of 6 cases with primary intestinal lymphoma, 4 cases appeared irreguar filling defect or truncation.Endoscopic performance: there were 5 cases of ulcer type and 3 cases of mass type in 13 gastric cases.Of 5 cases with primary intestinal lymphoma, 4 cases showed thickened mucosal folds, abnormal uplift, nodular or extensive erosion or irregular ulcers.Conclusion CT, gastrointestinal imaging and endoscopy have their own characteristics, comprehensive analysis of all kinds of inspection methods may improve the accuracy of diagnosis.%目的 探讨胃肠道淋巴瘤(PGIL)的多层螺旋CT、消化道造影及内镜检查特点,提高对该病诊断的准确性.方法 回顾性分析2011年10月~2015年4月经该院确诊的25例PGIL患者的CT图像、消化道造影及内镜检查特点,为临床早期诊断及治疗提供依据.结果 CT对PGIL诊断结果:14例胃部病例中,10例表现为胃壁弥漫性均匀增厚,2例表现为局限性胃壁增厚,呈结节样、息肉状改变,局部可见浅表样溃疡;11例肠道病例均表现为肠壁局限性或弥漫性增厚.造影显示:11例胃部病例中5例表现为弥漫性多发小结节样或肿块样充盈缺损,2例仅表现为黏膜皱襞不同程度的变平、增宽或破坏消失;6例肠道病例中,4例表现为不规则充盈缺损或截断.内镜检查:13例胃部病例中,溃疡型5例,肿块型3例;5例肠道病例中,4例表现为黏膜皱襞增厚,不规则隆起,结节样或广泛糜烂或不规则溃疡. 结论 CT、消化道造影及内镜检查对诊断PGIL具有各自不同的特点,应结合各种检查特点综合分析,提高诊断的准确性.
