目的 评价心脏磁共振在心血管病诊断方面的临床应用价值.方法 对我院自2015年5月~2016年2月成功进行的72例心脏磁共振检查,常规扫描序列包括基于自旋回波的HASTE黑血序列、基于梯度回波的TrueFISP亮血序列、基于梯度回波的多时相电影序列(TrueFISP Cine)、心肌首过灌注扫描(Turbo FLASH)、对比剂延迟增强扫描(PSIR Turb FLASH)等.结果 心脏磁共振扫描阳性病例66例,阳性率91.7%.阳性病例中包括缺血性心脏病23例,其中单纯心肌缺血,活性心肌尚存9例;陈旧性心肌梗死,无存活心肌14例,其中合并室壁瘤4例,存在无复流心肌2例,存在附壁血栓6例.扩张型心肌病7例.肥厚型心肌病5例,其中,心尖部肥厚型心肌病1例,伴左室流出道梗阻的肥厚型心肌病(梗阻性肥厚型心肌病)2例,无流出道梗阻的肥厚型心肌病(非梗阻性肥厚型心肌病)2例.高血压性心脏病11例,其中高血压性心脏病失代偿期8例,代偿期3例.左室心肌致密化不全3例.限制型心肌病1例.瓣膜性心脏病6例.心包炎1例.先天性心脏病3例.心脏肿瘤4例.主动脉瘤1例.心脏扫描过程中间接发现肺肿瘤(病理证实为腺癌)1例.结论 心脏磁共振的"一站式"扫描为心血管病的诊断提供了重要的帮助.%Objective To access the clinical value of magnetic resonance in the cardiovascular diseases.Methods A total of 72 patients with cardiovascular diseases from May 2015 to February 2016 in our hospital successfully underwent magnetic resonance scanning, including gradient Truefisp, TrueFISP Cine, Turbo FLASH, PSIR Turb FLASH, and etc.Results In the present study, 66 patients were found by magnetic resonance scanning, with positive rate of 91.7%.Those positive patients consisted of 23 patients with ischemicheart disease including myocardial viability (n=9) and old myocardial infarction (n=14), aneurysum (n=4), no-reflow myocardium (n=2), mural thrombus (n=6).The scanning also demonstrated the dilated cardiomyopathy (n=7), hypertrophic cardiomyopathy (n=5) including the apical hypertrophic cardiomyopathy (n=1), the obstructive hypertrophic cardiomyopathy (n=2), none of obstructive hypertrophic cardiomyopathy (n=2), high blood pressure (n=8) including compensatory period (n=3) and lossing compensation period (n=8), the left ventricular noncompaction (n=3) , the restrictive cardiomyopathy (n=1), the valvular disease (n=6), the pericarditis (n=1), the congenital heart disease (n=3), the heart tumor (n=4), and the aortic aneurysm (n=1).
