经口气管插管患者头颈位置变化对于套囊压力的影响

摘要

目的：：观察ICU经口气管插管患者头颈部位置变化对于套囊压力的影响，避免因套囊压力变化导致患者误吸或气道粘膜损伤，为科学管理套囊压力提供临床依据。方法：采用自身对照设计方案将某院重症医学科(ICU)经口气管插管患者160例为研究对象，患者正中体位为基准，采用套囊压力表分别测量患者头颈部弯曲、伸展、向左偏移、向右偏移后套囊压力，与正中体位时患者套囊压力比较。结果：患者头颈部弯曲后套囊压力增高，伸展后，向左偏移和向右偏移后套囊压力减低。结论：患者头颈部位置变化影响套囊压力，应及时调节，以减少套囊压力过高或过低导致相关并发症的发生率。%Objective:To observe the influence of head and neck position change of patients with breath tube intubation on cuff pressure,to avoid aspiration or airway mucosal inj ury due to capsule pressure changes and to provide clinical basis for scientific management of capsule pressure.Methods:160 cases of patients with breath tube intubation were selected as research obj ects using self-control contrast design,the mid-line position of patients was chosen as a benchmark,the capsule pressure were measured when head and neck of patients benched,stretched,shifted to the left or to the right,compare with the capsule pressure at the mid-line position.Results:Capsule pressure increased after bending of head and neck,capsule pressure decreased after stretching and shifting to the left or to the right.Conclusion:Head and neck position change of patients with breath tube intubation should be adj usted in time to reduce the incidence of related complications caused by cuff pressure.
