益生菌辅助糖皮质激素制剂治疗小儿湿疹的临床研究

             

摘要

Objective:To investigate the efficacy and mechanism of probiotics in the treatment of eczema.Methods:From April 2013 to March 2016,120 patients with eczema were selected and randomly divided into three groups according to the random number table method.Patients in the probiotics group were treated with Bifidobacterium vitiligo capsules.Patients in the glucocorticoid group were treated with hydrocortisone butyrate cream for topical treatment.The combined group was treated with probiotics and glucocorticoid.The levels of IFN-γ,IL-4,IL-10,IgE and Th1/Th2 were measured before and after treatment,and the intestinal bacteria of Bifidobacterium, Lactobacillus,Escherichia coli and Bifidobacterium/Escherichia coli Group analysis of the situation.Finally,the efficacy of the three groups of patients,the incidence of adverse reactions and the recurrence rate were recorded.Results: After treatment, the levels of IFN-γ,IL-4,IL-10,IgE and Th1/Th2 in the three groups were significantly improved (P<0.05),and the indexes of the combined group were significantly better than probiotics group and glucocorticoid group (P<0.05).The effect of intestinal flora in combination group was significantly better than that in probiotics group and glucocorticoid group,and the effect of Bifidobacterium, Lactobacillus and Bifidobacterium/Escherichia coli was significantly higher.There was no significant change in the level of Escherichia coli before and after treatment (P>0.05).The total effective rate (92.50%) in the combined group was significantly higher than that in the probiotics group (75.00%) and the glucocorticoid group (57.50%),and there was significant difference between the two groups (P<0.05).There was no significant difference in the incidence of adverse reactions between the three groups (P>0.05).The recurrence rate (5.00%) in the combined group was significantly lower than that in the probiotics group (20.00%) and the glucocorticoid group (27.50%),with statistically significant difference (P<0.05).Conclusion:Probiotics supplemented glucocorticoid preparation for the treatment of eczema,significant effect, good safety,it is worthy of extensive promotion.%目的:探讨益生菌辅助糖皮质激素制剂对湿疹治疗的疗效及机制.方法:选取2013年4月~2016年3月某院接诊的120例湿疹患者进行研究,按照随机数表法,随机分为3组.益生菌组患者应用双歧三联活菌胶囊对患者进行治疗,糖皮质激素组患者应用丁酸氢化可的松乳膏对患者进行外用治疗,联合组应用益生菌和糖皮质激素患者进行联合治疗.治疗前后检测记录3组患者的IFN-γ、IL-4、IL-10、IgE及Th1/Th2水平,并对双歧杆菌、乳酸杆菌、大肠杆菌及双歧杆菌/大肠杆菌的肠道菌群情况进行检测分析,记录分析3组患者的疗效、不良反应发生率及复发率.结果:治疗后,3组患者的IFN-γ、IL-4、IL-10、IgE及Th1/Th2水平均得到显著改善(P<0.05),且联合组患者治疗后的指标明显优于益生菌组和糖皮质激素组(P<0.05).3组患者的双歧杆菌、乳酸杆菌及双歧杆菌/大肠杆菌均显著升高(P<0.05),且联合组患者的肠道菌群改善效果明显优于益生菌组和糖皮质激素组,3组治疗前后大肠杆菌水平无明显变化(P>0.05).联合组治疗的总有效率(92.50%)明显高于益生菌组(75.00%)和糖皮质激素组(57.50%),组间比较有统计学意义(P<0.05).3组治疗后的不良反应发生率无显著性差异(P>0.05);联合组的复发率(5.00%)明显低于益生菌组(20.00%)和糖皮质激素组(27.50%),比较有统计学意义(P<0.05).结论:益生菌辅助糖皮质激素制剂治疗湿疹,疗效显著,安全性良好,值得广泛推广.
