牙科数字成像系统的准确性分析

     

摘要

目的:评价牙科数字成像系统(radio visio graphy,RVG)的准确性.方法:选择因正畸需要拔除的广东地区汉族青少年上颌第一前磨牙男女各100颗,按编号开髓,揭尽髓顶,每根管插-K锉至根尖孔处,利用RVG拍摄近远中向和颊舌向影像,按Vertucci标准对其根管分类.拍摄影像后的实验牙制作成透明标本,在体视显微镜下按Vertucci标准对其根管分类.评价RVG影像与体视显微镜下透明标本一致性:两名经过专业训练的研究生观察同一颗牙齿的颊舌向和近远中向影像和透明牙,按Vertucci的标准判断根管类型.最后采用SPSS13.0软件计算RVG影像和透明牙判断结果的一致性,即Kappa系数.结果:RVG影像结果与透明牙判断结果的Kappa值为0.573,符合中度一致性,具有统计学意义(P<0.001).结论:RVG影像对临床诊治有一定的参考价值.%Aim:To evaluate the accuracy of two-dimensional technique on studying of root and root canal morphology.Methods:The subjects composed of 200 Han nationality adolescents' maxillary first premolars extracted from female and male of Guangdong province' population for orthodontic therapy.The 200 maxillary first premolars were numbered in proper and after access preparation,a K-file was inserted into the canals until the tip of the file was just seen at the apical foramen.Then standardized periapical RadioVisioGraphy images were taken from mesial-distal and buccal-lingual directions.The 200 maxillary first premolars after taking RadioVisioGraphy from female and male separately were studied using a clearing technique by the teeth number.The cleared teeth were examined using a stereomicroscope.Both examinations of the root canal systems of the teeth were based on the Vertucci's classification.Evaluation of accuracy of the RVG images:The RVG images and the cleared teeth were evaluated and examined based on the root canal type by two independent trained postgraduate students.Results:The Kappa value was 0.573 between RVG images and the cleared teeth.Conclusion:The RVG images are helpful for clinical diagnosis and therapy.
