高位复杂性肛瘘术后外用溃疡散的临床观察

摘要

目的 通过对高位复杂性肛瘘术后换药的患者进行对比观察,了解外用溃疡散用于高位复杂性肛痿术后换药的效果和意义.方法 将2010年1月至2010年6月在院的符合纳入标准的高位复杂性肛瘘术后患者共32名按随机分组的原则,试验组16例采用外用溃疡散与复方紫草油纱条换药,对照组16例采用复方紫草油纱条换药,对两组的伤口疼痛、治疗时间及治疗效果等情况进行观察.结果 治疗时间试验组最短21 d,最长34 d,平均(25±2.3)d,对照组最短27 d,最长56 d,平均(33±4.1)d,两组差异有统计学意义(P＜0.05);疼痛方面试验组术后0级0例,Ⅰ级11例,Ⅱ级2例,Ⅲ级0例,对照组术后0级0例,Ⅰ级9例,Ⅱ级6例,Ⅲ级1例,两组差异有统计学意义(P＜0.05);两组均治愈16例,治愈率100%,两组相比差异无统计学意义(P＞0.05).临床观察发现试验组的创口分泌物少于对照组,肉茅鲜活,生长速度更快,换药后大部份患者感觉清凉舒适,疼痛程度明显轻于对照组.结论 高位复杂性肛瘘多切口挂线引流术后使用外用溃疡散换药可有效减少疼痛、促进创面生长、缩短愈合时间.%Objective To find out the effect and significance of applying Waiyong kuiyang san after surgical operations of high complex fistula-in-ano by comparing and observing two groups. Methods Thirty-two patients from 2010.1 to 2010.6 after surgical operations of high complex fistula-in-ano were randomly divided into two groups: 16 cases in the test group, 16 cases in the control group. Patients in the test group treating the wounds with Waiyong kuiyang san and Fufang zicao you, patients in control group treating the wounds with Fufang zicao you only. To compare pain, therapy time and therapy results between the two groups. Results The shortest therapy time of the test group was 21 d ,the longest one was 34d,the average therapy time (the par time of crae) were 25 ± 2.3d, The shortest therapy time of the control group was 27d ,the longest one was 56d,the average therapy time is 33 ±4.1d. The pain test of the test group:there were 0 case in level 0,1 lcases in level Ⅰ ,2 cases in level Ⅱ ,0 case in level Ⅲ; The pain test of the control group: there were 0 case in level 0,9cases in level Ⅰ ,6 cases in level Ⅱ ,1 case in level Ⅲ.Both of the cure rates of the two group were 100 percent. Compare the postoperative effect of the two groups,P＜0.05.the cuts of the patients of the test group were drier and growed faster than the control group,the patients of the test group feeled more comfortably than the control group. Conclusion Patients of high complex fistula-in-ano using with Waiyong kuiyang san after an operation by more than one incision and hanging drainage threads can get less pain and heal faster.
