舒张性心力衰竭患者左房重构及心功能与 CHADS2评分的相关性分析

摘要

目的：探讨舒张性心力衰竭患者左房重构及心功能与CHADS2评分的相关性及临床意义。方法：选择2013－2015年住院符合舒张性心力衰竭诊断标准、基础疾病为高血压病、冠心病、糖尿病患者258例，所有患者按CHADS2评分为低危组、中危组、高危组3组。心功能以血浆脑钠肽浓度表示；应用多普勒超声心动图检测左室射血分数（ LVEF ）、左室舒张末内径（ LVD）、左房内径（ LAD）、室间隔厚度（ IVSD）、左室后壁厚度（ LVPWD ）、E峰、A峰、E／A值多项指标，并同时测定患者血浆脑钠肽浓度。结果：随CHADS2评分增高，左房内径扩大，房颤比例升高，BNP值增高。结论：舒张性心力衰竭患者左房内径，房颤发生率及心功能与CHADS2评分有相关性。%Objective:To investigate the clinical relationship between CHADS2 score and left atrial remodeling and cardiac function in diastolic heart failure patients.Methods:258 cases of patients were selected,who were hospitalized from 2013-2015 for diseases of hypertension,coronary heart disease,diabetes complied with diastolic heart failure diagnostic criteria.All the cases, based on CHADS2 rating,were divided into a low-risk group,a risk group,and a high-risk group.The cardiac function was presented with plasma brain na-triuretic peptide concentration;Doppler echocardiography was employed to detect left ventricular ejection fraction( LVEF) ,left ventricu-lar end-diastolic diameter ( LVD) ,left atrial diameter( LAD) ,interventricular septum thickness ( ( IVST) ,left ventricular posterior wall thickness( LV PWT) ,E peak,A peak,E/A value,and plasma brain natriuretic peptide concentration was also measured.Results:With chads2 score increased,the left atrial diameter expanded,and also the atrial fibrillation proportion as well as BNP value rose.Conclu-sion:Chads2 score is correlated with LVD,the incidence of atrial fibrillation and cardiac function of diastolic heart failure patients.
